摘要
目的:探讨运动康复联合针灸对急性脑卒中偏瘫患者肢体运动能力和日常生活活动能力的影响。方法:将急性脑卒中偏瘫患者80例按照随机数字表法随机分为对照组和治疗组各40例,对照组在常规治疗的基础上给予运动康复疗法,治疗组在常规治疗的基础上给予运动康复疗法和针灸疗法,治疗4周后观察2组患者的临床疗效,同时采用四肢简化运动能力评分和改良Barthel指数对日常生活活动能力进行评定,并比较肌力情况。结果:治疗前2组患者运动能力、日常生活能力和肌力等比较差异无统计学意义(P>0.05)。治疗4周后,治疗组的总有效率为92.50%,高于对照组的72.50%,差异有统计学意义(χ~2=5.541,P=0.018)。治疗后2组患者运动能力评分、日常生活活动能力和神经功能缺损评分均高于治疗前,且治疗组评分高于对照组,差异有统计学意义(P<0.01)。2组患者治疗后肌力均有不同程度提高,治疗组患者肌力高于对照组(P<0.01)。结论:运动康复联合针灸可提高急性脑卒中偏瘫患者的临床疗效,改善患者日常生活活动能力,增强患者的运动能力,改善患者的肌力和神经功能缺损程度。
Objective： To explore the influence of sports rehabilitation jointed with acupuncture on limb motor ability and activities of daily living （ADL） of hemiplegia patients after acute stroke. Methods： Eighty patients were divided into the control group and the tzeatment group according to random number table method, 40 cases each group, the control group accepted sports rehabilitation therapy on the foundation of routine treatment, the treatment group were given sports rehabilitation therapy and acupuncture, clinical effects of the patients in both groups were observed after treating for four weeks, ADL was assessed by using simplified motor ability evaluation of the limbs and modified Barthel index, and muscle forces were compared. Results： The difference had no statistical meaning in the comparison of motor ability, ADL and muscle forces between both groups before treating （P〉0.05）. After treating for four weeks, total effective rate of the treatment group was 92.50%, higher than 72.50% of the control group, and the difference showed statistical meaning （Xz=5.541, P=0.018）. Motor ability scales, ADL and neurological severity score （NSS） of both groups after treating were higher than these before treating, and the treatment group was higher than the control group in the scales, and the difference showed statistical meaning （P^0.01）. After treating, muscle forces of both groups were improved to different degrees, the treatment group was higher than the control group in the muscle forces （P〈 0.01）. Conclusion： Sports rehabilitation jointed with acupuncture could raise clinical effects ofhemiplegia patients after acute stroke, ameliorate ADL of the patients, enhance motor ability, and improve muscle force and NSS.
