超声引导下腹横肌平面阻滞联合舒芬太尼静脉自控镇痛在结直肠癌术后患者中的应用观察 被引量:24

摘要 目的观察超声引导下腹横肌平面阻滞(TAP)联合舒芬太尼静脉自控镇痛(PCIA)在结直肠癌术后患者中的应用效果。方法择期行结直肠癌根治术患者48例,随机分为观察组与对照组各24例。两组均于气管内插管全身麻醉下进行结直肠癌根治术,术后观察组采用超声引导下TAP联合舒芬太尼PCIA,对照组采用舒芬太尼PCIA。评估两组术后2、4、8、12、24 h的疼痛强度VAS评分,记录术后24 h内舒芬太尼用量、第1次按压镇痛泵距离清醒拔管时间和镇痛泵按压次数。结果观察组术后2、4、8、12 h的VAS评分均明显低于对照组(P均<0.05)。观察组与对照组第1次按压镇痛泵距离清醒拔管时间分别为(9.5±2.1)、(4.4±2.3)h,24 h内患者按压镇痛泵次数分别为(6.5±4.0)、(15.6±7.4)次,舒芬太尼用量分别为(50.3±2.3)、(65.4±5.2)μg,两组比较,P均<0.05。观察组未见不良反应,对照组2例因按压镇痛泵次数较多,出现呕心、眩晕现象。结论超声引导下TAP联合舒芬太尼PCIA在结直肠癌术后患者中的应用效果较好,定位准确,镇痛效果好,安全性高。
