超声引导下腹横肌平面联合腹直肌鞘阻滞用于胃癌根治术后镇痛效果观察 被引量:10

摘要 目的探讨超声下引导腹横肌平面阻滞(TAPB)联合腹直肌鞘阻滞(RSB)在开腹胃癌根治术后的镇痛效果。方法行开腹胃癌根治术的患者60例,随机分为腹横肌平面联合腹直肌鞘阻滞组(TR组)、腹横肌平面阻滞组(T组)及空白对照组(C组),各20例。全身麻醉诱导后,在超声引导下TR组行TAPB+RSB,T组行TAPB,C组不行阻滞操作。3组术后均配用电子静脉自控式镇痛泵(PCIA)。记录3组术后0.5、2、4、8、16及24 h(分别对应设为T1~6)各时点疼痛评分(VAS)、镇静评分(Ramsay评分)及舒适度评分(BCS评分),记录术中瑞芬太尼使用量,记录术后24h PCIA按压次数及剩余药量。结果与C组比较,T组和TR组术后T1~5各时点的VAS评分降低、BCS评分升高(均<0.05),术后T2~5各时点的Ramsay评分明显降低(均<0.05);与T组相比,TR组T1~5各时点的VAS评分降低、BCS评分升高(均<0.05);与C组比较,T组和TR组PCIA按压次数明显降低、剩余量明显增加、术中瑞芬太尼用量明显减少(均<0.05);与T组相比,TR组PCIA按压次数显著降低、术中瑞芬太尼用量显著减少(均<0.05)。结论超声引导下TAPB联合RSB用于胃癌根治术具有镇痛效果好、并发症少及安全性高的特点,且可有效减少PCIA使用量,有利于患者快速康复,值得推广。
