摘要
2型糖尿病(T2DM)发病机制多认为由胰岛素抵抗(IR)和B细胞功能缺陷引起,而IR又是T2DM发生的基础,并贯穿于T2DM发生发展的全过程。IR的发生机制十分复杂,不仅与遗传因素高度相关,而且与胰岛素信号传导缺陷、多种脂肪源性细胞因子表达异常、物质代谢异常、炎症介质和氧化应激等诸多因素有关。但目前已发现的多种基因突变和诸多因素尚不能完全解释IR现象,各种因素致IR的确切机制和产生IR的许多环节还不十分清楚,IR不仅存在于外周组织中,B细胞自身也存在IR,并与T2DM相关。
Type 2 diabetes mellitus (T2DM) is the most common type of diabetes in diabetic population. At present, insulin resistance (IR) and B cell deficiency are the two basic pathogenesis of T2DM. IR may he the major factor and exists in the whole process of T2DM. Researches show that pathogenesis of IR is very complicated, is not only in high association with genetic factor, but also with insulin signal transduction defect, gene expression abnormality of adipose cell cytokine, abnormality of material metabolism, mediator of inflammation, oxidative stress and other factors. The gene mutations and many factors discovered can not explain the whole phenomenon of IR. The exact pathogenesis of IR caused by those factors and many links of IR are still unknown. Reports in recent years show that IR exists not only in peripheral tissue but also in B cell of pancreas islet, and the IR of B cell is related to T2DM. Based on the literature at home and abroad, this article reviewed the pathogenesis of IR and the relationship between IR and T2DM.
