摘要
目的观察老年全髋关节置换术患者采用功能锻炼结合SuperPath微创后入路对其康复的影响.方法选取2016年2月—2021年2月于我院行全髋关节置换术的患者96例为研究对象,采用抽签法将其随机分为对照、观察两组,每组48例.两组均进行功能锻炼,对照组开展全髋关节置换术时经后外侧入路,观察组则经SuperPath微创后入路.观察并比较两组患者的手术指标及康复情况.结果观察组的住院时间、首次下床时间均短于对照组,术中出血量少于对照组,并发症发生率低于对照组,组间差异有统计学意义(P<0.05).术前,两组的疼痛视觉模拟评分(VAS)、Berg平衡量表(BBS)评分、Harris髋关节功能评分量表(HHS)评分及健康调查简表(SF-36)评分比较,组间差异无统计学意义(P>0.05);术后1个月,观察组的VAS评分为(1.11±0.35)分,低于对照组,BBS评分、HHS评分、SF-36评分分别为(48.72±4.73)分、(87.72±5.33)分、(90.43±3.21)分,均高于对照组,组间差异有统计学意义(P<0.05).结论功能锻炼联合SuperPath微创后入路可有效加快老年全髋关节置换术患者的康复速度,利于改善其预后,值得临床推广应用.
Objective To observe the effect of functional exercise combined with SuperPath minimally invasive posterior approach on rehabilitation of elderly patients undergoing total hip arthroplasty.Methods A total of 96 patients who underwent total hip arthroplasty in our hospital from February 2016 to February 2021 were selected as the research objects,and they were randomly divided into a control and an observation group by drawing lots,with 48 patients in each group.The functional exercises were performed before and after operation in both groups.Total hip arthroplasty was performed via posterolateral approach in the control group while minimally invasive SuperPath approach in the observation group.The operation indexes and recovery of the two groups were observed and compared.Results In terms of surgical indicators,the length of hospitalization and the time to get out of bed for the first time in the observation group were shorter than those in the control group,the intraoperative bleeding was less than that in the control group,and the incidence of complications was lower than that in the control group,the differences between the groups were statistically significant(P<0.05).Before surgery,there were no significant differences in visual analogue scale score(VAS),Berg balance scale(BBS)score,Harris hip scale(HHS)score,and the MOS item short from health survey(SF-36)score between the two groups(P>0.05);1 month after surgery,the VAS score of the observation group was(1.11±0.35)points,lower than that of the control group,the BBS score,HHS score and SF-36 score were(48.72±4.73)points,(87.72±5.33)points,(90.43±3.21)points,respectively,higher than those of the control group,the differences between groups were statistically significant(P<0.05).Conclusion Functional exercise combined with SuperPath minimally invasive posterior approach can effectively accelerate the recovery speed of elderly patients with total hip arthroplasty,which is beneficial to improve their prognosis and worthy of clinical application.
