摘要
目的调查分析老年全髋关节置换术(THA)患者术后并发下肢深静脉血栓(DVT)的独立危险因素。方法收集2019年9月-2020年9月期间深圳市龙岗中心医院收治的老年THA患者86例。收集老年患者的基础资料后,统计分析86例老年THA患者术后并发下肢DVT情况;分析THA患者并发下肢DVT的危险因素的单因素;以线性Logistic回归方程分析THA患者并发下肢DVT的危险因素的多因素。结果调查发现86例老年患者中,THA术后并发下肢DVT25例,发生率为29.07%。老年THA患者并发下肢DVT的危险因素单因素为高血压、糖尿病、伴心血管病症、术后卧床时间>4d、麻醉方式、近3年患有血栓、护理方式、术后2d D-二聚体(D-D)水平及甘油三酯(TG)水平(P<0.05);糖尿病、伴心血管病症、术后卧床时间>4d、近3年患有血栓及术后2d D-D水平为老年THA患者并发下肢DVT的独立危险性因素(P<0.05),而术后快速康复护理是老年THA患者并发下肢DVT的保护性因素(P<0.05)。结论经研究发现糖尿病、伴心血管病症、术后卧床时间>4d、近3年患有血栓及D-二聚体(D-D)水平为老年THA患者并发下肢DVT的独立危险性因素,针对上述因素,加强护理干预,以降低老年THA患者术后并发下肢DVT的风险。
Objective To investigate and analyze independent risk factors of lower extremity deep venous thrombosis(DVT)in elderly patients after total hip replacement(THA).Methods The paper chose 86 elderly THA patients admitted to Shenzhen Longgang Central Hospital from September 2019 to September 2020.After collecting basic data,86 elderly THA patients with postoperative DVT of lower limbs were statistically analyzed;single factor of risk factors of THA patients complicated with lower limb DVT were analyzed;multifactorial risk factors of THA patients complicated with lower extremity DVT was analyzed with linear logistic regression equation.Results Among 86 elderly patients,there were 5 cases of lower limb DVT25 after THA,with incidence of 29.07%.Single risk factors of lower limb DVT of elderly THA patients were hypertension,diabetes,cardiovascular disease,postoperative bed rest time>4d,anesthesia mode,thrombosis in recent 3 years,nursing mode,postoperative 2d D-Dimer(D-D)level and triglyceride(TG)level,(P<0.05).Diabetes,cardiovascular disease,postoperative bed rest time>4d,thrombosis in recent 3 years and postoperative 2d D-D level were independent risk factors for elderly THA patients complicated with lower limb DVT,(P<0.05),while postoperative rapid rehabilitation nursing was protective factor for elderly THA patients complicated with lower limb DVT,(P<0.05).Conclusion Study found that diabetes,cardiovascular disease,postoperative bed rest time>4d,thrombosis in recent 3 years and D-Dimer(D-D)level are independent risk factors of lower limb DVT of elderly THA patients.in view of above factors,it is necessary to strengthen nursing intervention to reduce risk of lower limb DVT of elderly THA patients.
