阶梯性康复训练结合静态牵伸训练对桡骨远端骨折患者腕关节功能的影响 被引量:1

Effect of Step Rehabilitation Training Combined with Static Stretching Training on Wrist Function in Patients with Distal Radius Fracture

摘要 目的探讨阶梯性康复训练结合静态牵伸训练对桡骨远端骨折患者腕关节功能的影响.方法选取2020年1月—2022年2月我院收治的70例桡骨远端骨折患者为研究对象,根据随机数字表法分为对照组(n=35)和观察组(n=35).对照组采用常规康复训练,观察组采用阶梯性康复训练结合静态牵伸训练,均持续训练2个月.比较两组腕关节功能、腕关节活动度、骨折恢复情况.结果训练前,两组Cooney腕关节功能评分量表各维度评分、腕关节各方向活动度比较,组间差异无统计学意义(P>0.05);训练后,观察组Cooney腕关节功能评分量表中的疼痛评分(21.34±1.83)分、功能评分(21.21±1.71)分、握力评分(21.35±1.41)分、活动范围评分(21.36±1.37)分、屈曲/伸展活动度评分(21.29±1.35)分均高于对照组,组间差异有统计学意义(P<0.05);观察组腕关节背伸度(47.29±2.41)°、掌屈度(54.68±2.24)°、桡偏角度(16.14±2.21)°、旋前度(86.34±2.31)°、旋后度(86.58±2.91)°均大于对照组,组间差异有统计学意义(P<0.05);观察组疼痛消失时间(24.27±2.11)d、临床愈合时间(79.07±2.18)d、骨性愈合时间(114.21±2.16)d均短于对照组,组间差异有统计学意义(P<0.05).结论阶梯性康复训练结合静态牵伸训练可促进桡骨远端骨折患者腕关节功能恢复,增加腕关节活动度,缩短骨折愈合时间. Objective To investigate the effect of step rehabilitation training combined with static stretching training on wrist function in patients with distal radius fracture.Methods 70 patients with distal radius fracture admitted to the hospital from January 2020 to February 2022 were selected as the research subjects,and were divided into a control group(n=35)and an observation group(n=35)according to random number table.The control group received routine rehabilitation training,while the observation group received step rehabilitation training combined with static stretching training,both groups continued training for 2 months.The wrist function,wrist range of motion and fracture recovery were compared between the two groups.Results Before training,there was no significant difference in the score of each dimension of Cooney wrist function rating scale and range of motion in all directions of the wrist between the two groups(P>0.05);after training,the scores of pain,function,grip strength,range of motion and flexion/extension range of motion in the Cooney wrist function scale of the observation group were(21.34±1.83)points,(21.21±1.71)points,(21.35±1.41)points,(21.36±1.37)points,(21.29±1.35)points,respectively,which were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05);the dorsi extension angle,palmar flexion angle,radial deviation angle,pronation angle and supination angle of the wrist in the observation group were(47.29±2.41)°,(54.68±2.24)°,(16.14±2.21)°,(86.34±2.31)°and(86.58±2.91)°,respectively,which were greater than those in the control group,and the differences between the groups were statistically significant(P<0.05);the pain disappearance time,clinical healing time and bone healing time of the observation group were(24.27±2.11)d,(79.07±2.18)d and(114.21±2.16)d,respectively,which were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Step rehabilitation training combined with static stretching training can promote functional recovery of the wrist in patients with distal radius fractures,increase the range of motion of the wrist and shorten the healing time of the fracture.
