摘要
目的探究脊柱侧凸患者采用斜外侧腰椎椎间融合术(OLIF)治疗的效果。方法选择2018年6月—2022年8月本院收治的78例脊柱侧凸患者为研究对象,按随机数字表法分为两组,每组39例。对照组行后路腰椎椎间融合术(PLIF),观察组行OLIF。比较两组患者的围术期指标、腰腿疼痛情况、腰椎功能及并发症发生情况。结果两组的手术时间比较,差异无统计学意义(P>0.05);观察组的术中出血量为(159.43±16.30)mL,少于对照组的(216.83±20.17)mL,切口长度、术后下床活动时间、住院时间分别为(6.31±0.67)cm、(1.51±0.11)d、(12.48±1.33)d,均短于对照组的(10.87±1.02)cm、(2.72±0.23)d、(15.36±1.26)d,组间差异有统计学意义(P<0.05)。术后3个月,观察组的腰部疼痛视觉模拟评分(VAS)、腿部VAS评分、Oswestry功能障碍指数问卷表评分分别为(2.06±0.28)分、(1.91±0.25)分、(10.67±1.32)分,均低于对照组的(4.20±0.37)分、(3.90±0.37)分、(11.80±1.56)分,日本骨科协会评估表评分为(19.05±2.32)分,高于对照组的(17.56±2.11)分,组间差异有统计学意义(P<0.05)。两组的并发症发生率比较,差异无统计学意义(P>0.05)。结论与PLIF比较,采用OLIF治疗脊柱侧凸患者能更为有效地促进术后恢复,减轻腰腿疼痛,改善腰椎功能,且并发症较少。
Objective To investigate the effects of oblique lateral lumbar interbody fusion(OLIF)in patients with scoliosis.Methods 78 patients with scoliosis admitted to the hospital from June 2018 to August 2022 were selected as the research objects and were divided into two groups according to random number table method,with 39 cases in each group.The control group underwent posterior lumbar interbody fusion(PLIF),and the observation group underwent OLIF.The perioperative indexes,lumbar and leg pain,lumbar function and complications were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05);the intraoperative blood loss in the observation group was(159.43±16.30)mL,less than(216.83±20.17)mL in the control group,the incision length,postoperative ambulation time and hospital stay were(6.31±0.67)cm,(1.51±0.11)d and(12.48±1.33)d,respectively,which were shorter than(10.87±1.02)cm,(2.72±0.23)d and(15.36±1.26)d in the control group,and the differences between the groups were statistically significant(P<0.05).3 months after surgery,the low back pain visual analogue scale(VAS)score,leg VAS score and Oswestry disability index score of the observation group were(2.06±0.28)points,(1.91±0.25)points and(10.67±1.32)points,respectively,which were lower than(4.20±0.37)points,(3.90±0.37)points,(11.80±1.56)points in the control group,and the Japanese Orthopaedic Association evaluation score was(19.05±2.32)points,higher than(17.56±2.11)points in the control group,the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Compared with PLIF,OLIF is more effective in promoting postoperative recovery,alleviating lumbar pain and improving lumbar function in patients with scoliosis with fewer complications.
