摘要
目的:探讨益气化瘀汤配合微创经椎间孔腰椎椎间融合术(MIS-TLIF)手术治疗腰椎退行性疾病的临床疗效。方法:将2021年2月至2022年2月我院收治的147例腰椎退行性病变患者按照随机数字表法分为对照组73例和观察组74例,两组均予MIS-TLIF手术治疗,对照组术后予甲钴胺配合治疗,观察组则配合益气化瘀汤治疗,记录两组临床疗效、中医证候积分、腰椎功能恢复情况以及治疗前后炎症介质水平变化。结果:治疗2月后,观察组与对照组临床有效率分别为98.65%、89.04%,观察组高于对照组(P<0.05)。观察组中医证候积分低于对照组(P<0.05)。观察组日本骨科协会评分表(JOA)评分评分高于对照组(P<0.05),Oswestry功能障碍指数(ODI)及视觉模拟评分法(VAS)评分均低于对照组(P<0.05)。观察组超敏C反应蛋白(hs-CRP)、白细胞介素-1α(IL-1α)及肿瘤坏死因子-α(TNF-α)水平均低于对照组(P<0.05)。结论:益气化瘀汤配合MIS-TLIF手术治疗腰椎退行性病变,有利于提高临床疗效,改善患者临床不适症状,减轻术后炎症反应,促进患者腰椎功能恢复。
Objective:To explore the clinical efficacy of applying Yiqi Huayu Decoction combined with minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)in the treatment of lumbar degenerative diseases.Methods:147 patients with lumbar degenerative diseases who were treated in the hospital between February 2021 and February 2022 were selected as the research subjects,and they were divided into a controlled group(73 cases)and an observation group(74 cases)according to the random number table method.Both groups were treated with MIS-TLIF surgery,and the controlled group was given Mecobalamin after surgery while the observation group was combined with Yiqi Huayu Decoction.The clinical efficacy,scores of TCM syndromes,lumbar function recovery and levels of inflammatory mediators before and after treatment were recorded in the two groups.Results:After 2 months of treatment,the clinical effective rate of the observation group was higher than that of the controlled group(P<0.05).The TCM syndromes scores of the observation group were lower compared with those of the controlled group(P<0.05).The Japanese Orthopaedic Association(JOA)score in the observation group was higher(P<0.05)while the scores of Oswestry Disability Index(ODI)and Visual Analogue Scale(VAS)were lower(P<0.05)compared to the controlled group.The levels of hypersensitive C-reactive protein(hs-CRP),interleukin-1α(IL-1α)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the controlled group(P<0.05).Conclusion:Yiqi Huayu Decoction combined with MIS-TLIF surgery in the treatment of lumbar degenerative diseases is beneficial to enhance the clinical efficacy,improve the clinical symptoms,relieve the postoperative inflammatory response and promote the recovery of lumbar function.
