气压治疗仪在脑卒中偏瘫患者下肢深静脉血栓预防治疗中的临床研究 被引量:5

摘要 目的:探讨脑卒中偏瘫患者予以气压治疗仪治疗的效果及对下肢深静脉血栓的预防状况。方法:研究样本从本院脑卒中偏瘫患者中选取,共86例,研究时间介于2022年5月-2023年5月之间,采用随机数字表分为对照组(常规治疗)与观察组(气压治疗仪治疗),各43例。观察患者溶栓时间、尿激酶用量、血栓清除率、下肢深静脉血栓发生情况、凝血指标、血流峰速以及神经功能等指标水平变化情况。结果:治疗后,相较于对照组,观察组溶栓时间、尿激酶用量与DVT发生率均更低,血栓清除率更高(P<0.05);观察组PT、APTT、FIB等水平均显著更优(P<0.05);观察组腘静脉血流峰速度、股静脉血流峰速度以及平均血流速度均较对照组更高,而NIHSS评分则较对照组更低(P<0.05)。结论:脑卒中偏瘫患者临床治疗中选用气压治疗仪治疗方案,可明显提高患者血栓清除率,并可改善其凝血功能,提升血流速度,有效降低下肢深静脉血栓发生风险,提升患者神经功能恢复效果,获得更理想的治疗有效性。 Objective:To investigate the effect of barometric pressure therapy on stroke patients with hemiplegia and the prevention of lower extremity thrombosis.Methods:A total of 86 stroke patients with hemiplegia were enrolled in the study,which was conducted between May 2022 and 2023 May,the patients were randomly divided into two groups:the control group(routine treatment)and the observation group(treatment with barometric pressure therapeutic instrument),43 cases in each group.The time of thrombolysis,urokinase dosage,thrombectomy rate,lower extremity thrombosis,coagulation parameters,peak blood flow velocity and neurological function were observed.Results:Compared with the control group,the thrombolytic time,the dosage of urokinase and the incidence of DVT in the observation group were lower,the thrombus clearance rate was higher(p<0.05),the levels of PT,APTT and FIB in the observation group were significantly better(p<0.05)The peak velocity of popliteal vein,femoral vein and mean velocity of blood flow in the observation group were higher than those in the control group,and the NIHSS score was lower than that in the control group(p<0.05).Conclusion:In the clinical treatment of stroke patients with hemiplegia,the selection of the therapeutic scheme of the air pressure therapeutic instrument can obviously increase the thrombus clearance rate,improve the blood coagulation function and increase the blood flow velocity,it can effectively reduce the risk of thrombosis in lower limbs,improve the recovery of neurological function,and obtain more ideal therapeutic effectiveness.
