摘要
目的探讨三阴性乳腺癌(TNBC)患者超声表现及其与细胞角蛋白5/6(CK5/6)、BRCA1蛋白的关系。方法选取自2018年1月至2021年12月在北部战区总医院手术且经病理证实的165例乳腺癌(单发肿瘤)女性患者为研究对象。根据雌激素受体、孕激素受体和人类表皮生长因子受体2的表达结果将患者分为TNBC组(n=62)与非TNBC组(n=103)。患者均采取手术治疗,取术中切除的乳腺癌组织,采用免疫组织化学法检测CK5/6和BRCA1的表达水平。分析TNBC患者超声表现,以及与CK5/6、BRCA1表达的关系。结果TNBC组肿块边缘清晰、后方回声增强以及呈富血供的比例均显著高于非TNBC组,发生微钙化的比例明显低于非TNBC组,差异均有统计学意义(P<0.05)。CK5/6表达阳性者的纵横比≤1、边缘清晰、形态规则比例显著高于阴性者,差异有统计学意义(P<0.05)。BRCA1表达阴性者后方回声增强和富血供比例明显高于阳性者,差异有统计学意义(P<0.05)。结论TNBC超声表现与其他分子分型乳腺癌具有明显差异,CK5/6、BRCA1表达与TNBC超声表现有一定关系。
Objective To investigate the relationship between the ultrasonographic manifestations and cytokeratin 5/6(CK5/6)and BRCA1 protein in patients with triple negative breast cancer(TNBC).Methods A total of 165 pathologically confirmed female breast cancer(single tumor)patients who underwent surgery in General Hospital of Northern Theater Command from January 2018 to December 2021 were selected as the study subjects.According to the expression of estrogen receptor,progesterone receptor and human epidermal growth factor receptor 2,patients were divided into TNBC group(n=62)and non-TNBC group(n=103).All patients underwent surgical treatment,and the breast cancer tissues removed during surgery were collected,and the expression levels of CK5/6 and BRCA1 were detected by immunohistochemistry.To analyze the ultrasound manifestations of TNBC patients and their relationship with CK5/6 and BRCA1 expression.Results The proportion of clear tumor margin,enhanced posterior echo and rich blood supply in the TNBC group was significantly higher than that in the non-TNBC group,and the proportion of microcalcification was significantly lower than that in the non-TNBC group,with statistical significance(P<0.05).CK5/6 positive patients had significantly higher aspect ratio≤1,clear edge and regular morphology than negative patients,with statistical significance(P<0.05).The proportion of posterior echo enhancement and rich blood supply in BRCA1 negative patients was significantly higher than that in BRCA1 positive patients,with statistical significance(P<0.05).Conclusion The ultrasound performance of TNBC was significantly different from that of other molecular types of breast cancer.The expression of CK5/6 and BRCA1 is related to the expression of TNBC ultrasound.
