乳腺癌患者120例临床超声弹性成像评分、病理特征分析 被引量:3

摘要 目的:探究乳腺癌患者的超声弹性成像评分及病理学特征分析。方法:将我院于2016年8月~2019年10月收治的120例乳腺癌患者作为研究对象,所有患者在术前均接受超声弹性成像检查。比较不同肿瘤分级、不同分子亚型乳腺癌患者的各弹性成像定量参数,就超声弹性成像面积比、应变率比值以及E mean、E radio值与乳腺癌组织病理学特征的关系进行分析。结果:超声弹性成像检查显示,乳腺癌肿瘤组织多不规则,呈椭圆形,肿瘤边界模糊,内部多表现为低回声。肿瘤分级越高的患者,各项弹性成像定量参数值越高,不同分级乳腺癌的各项弹性成像定量参数比较差异有统计学意义。不同分子亚型乳腺癌患者中,ERBB2+型的各弹性成像定量参数值最高,Luminal-like型次之,Basal-like型最低,组间差异比较有统计学意义。不同ER、PR、HER2及Ki-67表达患者的面积比、应变率比值、E mean、E radio值比较无统计学意义,不同肿瘤直径、肿瘤类型、肿瘤分级、分子亚型患者的面积比、应变率比值、E mean、E radio值之间存在显著差异,差异比较有统计学意义。结论:乳腺癌肿瘤组织硬度与其病理学特征存在一定相关性,利用超声弹性成像各定量参数评分能够对肿瘤组织硬度进行全面评估,从而为其病情评估及临床治疗提供重要参考。 Objective To research the ultrasonic elastography score and pathological characteristics of breast cancer patients.Methods 120 breast cancer patients admitted to our hospital from August 2016 to October 2019 were taken as the study objects.All patients were examined by ultrasound elastic imaging before operation.The quantitative parameters of elastic imaging of breast cancer patients with different tumor grades and molecular subtypes were compared.The relationship between the area ratio,strain rate ratio,E mean,E radio and the histopathological characteristics of breast cancer were analyzed.Results The ultrasonic elastic imaging showed that most of the breast cancer tissues were irregular,oval,with fuzzy tumor boundary and low echo.The higher the tumor grade was,the higher the quantitative parameters of elastic imaging was.There was a significant difference between the quantitative parameters of elastic imaging in different grades of breast cancer.Among the patients with different molecular subtypes of breast cancer,the quantitative parameters of elastic imaging of ERBB2+type were the highest,luminal like type was the second,and basal like type was the lowest.There was no significant difference in the area ratio,strain rate ratio,emean and e-radio values among the patients with different ER,PR,HER2 and Ki-67 expression.There were significant differences in the area ratio,strain rate ratio,emean and e-radio values among the patients with different tumor diameter,tumor type,tumor grade,molecular subtype Conclusion The tumor tissue hardness of breast cancer is closely related to its pathological characteristics.The quantitative parameters of ultrasound elastography can be used to comprehensively evaluate the tumor tissue hardness.It provides an important reference for the evaluation and clinical treatment of the disease.
