摘要
目的探究通筋透骨汤结合聪脑通络法针刺促进痉挛性脑瘫患儿康复效果。方法研究合计纳入该院2018年8月—2020年9月康复科收治的93例痉挛性脑瘫患儿,采取随机数字表法将纳入患儿分成两组,对照组47例患儿均行常规治疗,观察组46例患儿常规治疗+通筋透骨汤结合聪脑通络法针刺治疗,比较两组患儿疗效、治疗前后血清脑源性神经营养因子(BDNF)水平变化及智力发育(GDS评分)情况、治疗前后三维步态时空指数变化情况、改良Ashworth痉挛评定量表评分变化及粗大运动功能量表(GMFM)评分变化、日常生活能力(ADL)量表评分变化、残疾程度评分变化及不良反应情况。结果较对照组(82.98%,39/47),观察组患儿有效率更高(95.65%,44/46)(P<0.05);治疗前,两组患儿BDNF水平、GDS评分、三维步态时空指数、改良Ashwoeth痉挛评定量表评分、GMFM评分及ADL评分、残疾程度评分等指标比较(P>0.05),治疗后各组患儿BDNF水平、GDS评分、三维步态时空指数、改良Ashworth痉挛评定量表评分、GMFM评分及ADL评分、残疾程度评分等指标均改善,观察组患儿治疗后BDNF水平、GDS评分、三维步态时空指数、改良Ashworth痉挛评定量表评分、GMFM评分及ADL评分、残疾程度评分等指标优于对照组患儿(P<0.05);对照组患儿未见不良反应,观察组出现1例针刺皮下血肿、硬结(P>0.05)。结论通筋透骨汤结合聪脑通络法针刺可较好促进痉挛性脑瘫患儿康复,患儿智力发育好,症状改善,运动功能恢复好,可一定程度改善患儿残疾情况,患儿日常生活能力更佳,不良反应少,安全性较高,值得应用。
Objective To explore the effect of Tongjin Tougu Decoction(通筋透骨汤)combined with tonifying brain and dredging collaterals acupuncture in promoting the rehabilitation of children with spastic cerebral palsy.Methods A total of 93 children with spastic cerebral palsy from August 2018 to September 2020 were included in the study.The children were divided into two groups by random number table method.The control group(47 children)received conventional treatment and the observation group(46 children)received conventional treatment combined with Tongjin Tougu Decoction combined with tonifying brain and dredging collaterals acupuncture.The curative effect,the changes of serum brain-derived neurotrophic factor(BDNF)and intellectual development(GDS score)before and after treatment,the changes of three-dimensional gait space-time index before and after treatment,the changes of modified Ashworth spasticity rating scale,gross motor function scale(GMFM),ability of daily living(ADL)and degree of disability and adverse reactions were compared between the two groups.Results Compared with that of the control group(82.98%,39/47),the effective rate of the observation group was higher(95.65%,44/46)(P<0.05).Before treatment,BDNF level,GDS score,three-dimensional gait spatio-temporal index,modified Ashwood spasm Rating Scale score,GMFM score,ADL score,disability score and other indicators were not significantly different between the two groups(P>0.05).After treatment,BDNF level,GDS score,three-dimensional gait spatio-temporal index,modified Ashworth spasm Rating Scale score,GMFM score,ADL score,disability score and other indicators were improved and those in the observation group were better than those in the control group(P<0.05).There was no adverse reaction in the control group and there was 1 case of acupuncture subcutaneous hematoma and induration in the observation group(P>0.05).Conclusion Tongjin Tougu Decoction combined with tonifying brain and dredging collaterals acupuncture can better promote the rehabilitation of children with spastic cerebral palsy.The children have good intellectual development,improved symptoms and good recovery of motor function.It can improve the disability of children to a certain extent.The children have better ability of daily living,fewer adverse reactions and high safety.It is worthy of application.
