摘要
目的:分析黄连温胆汤联合吴茱萸穴位贴敷治疗对脑卒中后睡眠障碍患者的临床效果。方法:收集本院2019年1月~2020年11月收治的88例脑卒中后睡眠障碍患者的临床资料。根据不同治疗方法分为对照组(n=42例,右佐匹克隆)与研究组(n=46例,黄连温胆汤剂联合吴茱萸穴位贴敷),比较两组临床疗效、匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PQSI)评分、汉密尔顿焦虑(Hamilton Anxiety,HAMA)评分、汉密尔顿抑郁(Hamilton Depression,HAMD)评分、中医证候积分、血清脑源性神经营养因子(Brain-derived Neurotrophic Factor,BDNF)与5-羟色胺水平、生活质量及不良反应发生情况。结果:研究组治疗后总有效率为93.48%,显著高于对照组的78.57%(P<0.05)。治疗后,两组PQSI、HAMA、HAMD评分、中医证候积分及ADL评分均较治疗前降低,且研究组降低程度显著优于对照组(P<0.05)。两组治疗后MBL评分、BDNF及5-羟色胺水平均较治疗前上升,以研究组上升更为显著(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:黄连温胆汤剂联合吴茱萸穴位贴敷治疗脑卒中后睡眠障碍患者疗效确切,可改善患者中医证候、提高睡眠质量,缓解焦虑及抑郁情绪,进而改善患者生活质量,安全可靠。
Objective:To analyze the clinical effect of using Huanglian Wendan Decoction combined with evodia acupoint application in the treatment of patients with sleep disorders after stroke.Methods:The clinical data of 88 patients with sleep disorders after stroke admitted to our hospital from January 2019 to November 2020 were collected.According to different treatment methods,they were divided into a controlled group(n=42 cases,Dexzopiclone)and a study group(n=46 cases,Huanglian Wendan Decoction combined with Evodia acupoint application).The clinical efficacy,Pittsburgh sleep quality index(PQSI)score,Hamilton Anxiety(HAMA)score,Hamilton Depression(HAMD)score,TCM syndrome score,brain-derived neurotrophic factor(BDNF)and serotonin levels,quality of life and occurrence of adverse reactions between the two groups were compared.Results:The total effective rate after treatment in the study group was 93.48%,which was significantly higher than the 78.57%in the controlled group(P<0.05).After treatment,the PQSI,HAMA,HAMD scores,TCM syndrome scores and ADL scores in the two groups were lower than those before treatment,and the reduction in the study group was significantly better than that in the controlled group(P<0.05).The MBL score,BDNF and serotonin levels of the two groups increased after treatment compared to those before treatment,and the increase was more significant in the study group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Huanglian Wendan Decoction combined with Evodia acupoint application has a definite effect in the treatment of patients with sleep disorders after stroke.It can improve TCM syndromes,improve sleep quality,relieve anxiety and depression,and improve the quality of life of patients.It is safe and reliable.
