136例不同成分用血的输血不良反应回顾性分析 被引量:4

摘要 目的为了解不同血液成分对输血不良反应的影响,为临床用血安全提供依据。。方法统计某三甲医院各临床科室2014年1月1日-2019年9月20日不同成分用血的输血不良反应共136例,运用SPSS22.0统计软件对输注悬浮红细胞、去白(少白)细胞悬浮红细胞、病毒灭活血浆、新鲜冰冻血浆、冰冻血浆、机采血小板和冷沉淀不良反应率进行数据分析,采用卡方检验比较输注不同成分血不良反应率的差异。结果发现输注新鲜冰冻血浆比病毒灭活血浆不良反应比例高,且差异显著P=0.003,OR值为11.321,95%可信区间为1.533~83.596,说明新鲜冰冻血浆是引发输血不良反应的一个危险因素。输注冷沉淀不良反应比例比红细胞低,差异显著P=0.000,OR值为0.166,95%可信区间为0.060~0.457,说明红细胞比冷沉淀相更易引发输血不良反应。另外血浆和血小板分别与红细胞相比不良反应比例差异不显著,去白(少白)细胞悬浮红细胞与悬浮红细胞相比不良反应比例差异不显著,冰冻血浆与病毒灭活血浆相比不良反应比例差异不显著。结论给患者输注成分血时,应把握输血适应症,合理选择成分血输注,在保证输血疗效的同时减少输血不良反应的发生。 Objective To understand the effects of different blood components on adverse reactions of transfusion.Methods The adverse reactions of blood transfusion with different components in the clinical departments of a Three A and Tertiary Hospital from January 1 st,2014 to September 20 th,2019,and the data of the infusion of suspended red blood cells,Leukocyte-depleted(or less white cell)suspended red blood cells,virus inactivated plasma,fresh frozen plasma,frozen plasma,plateletpheresis and cryoprecipitate adverse reaction rate were analyzed by SPSS 22.0.Chi-square test was used to compare the difference of blood adverse reaction rate of different components blood.Results This study found that the ratio of adverse reactions of fresh frozen plasma was higher than virus inactivated plasma,and the difference was significant P=0.003,OR value was 11.321,95%confidence interval was 1.533~83.596,indicating that fresh frozen plasma was a risk factor for adverse reactions of blood transfusion.The proportion of adverse reactions infusion of cryoprecipitate was lower than red blood cells,the difference was significant P=0.000,the OR value was 0.166,and the 95%confidence interval was 0.060~0.457,indicating that red blood cells were more likely to cause adverse reactions of transfusion than cryoprecipitate.In addition,plasma and platelets had no significant difference in the proportion of adverse reactions compared with red blood cells,and there was no significant difference in the proportion of adverse reactions between red blood cells(removed white blood cells)and red blood cells,there was no significant difference in the proportion of adverse reactions between frozen plasma and virus inactivated plasma.Conclusions When blood components transfusion into patients,we should grasp the indications for transfusion and rationally select blood transfusions to reduce the adverse reactions of transfusion while ensuring the efficacy of transfusion.
