摘要
目的探究剖宫产对术后再次妊娠产妇经阴道分娩产程的影响。方法佛山市南海区人民医院在2017年1月-2020年12月间选择80例剖宫产术后再次妊娠符合自然分娩条件的产妇归入研究组,同期选择足月自然临产的40例初产妇作为对比组,两组均未实施人工破膜、宫缩素等干预,比较两组产程时间以及侧切率、产后出血发生率、撕裂率、产褥感染率以及新生儿Apgar评分、新生儿脐动脉pH值、新生儿呼吸窘迫综合征情况。结果与对比组相比,研究组的第一产程、第二产程、总产程时间更短(P<0.05)。研究组的侧切率、撕裂率、产后出血发生率、产褥感染率与对比组相比,差异无统计学意义(P>0.05)。研究组新生儿呼吸窘迫综合征发生率、新生儿脐动脉pH值<7.0率、新生儿Apgar评分<7分率与对比组相比,差异无统计学意义(P>0.05)。结论剖宫产术后再次妊娠产妇若符合阴道分娩条件,可选择自然分娩,其第一产程、第二产程、总产程时间更短于初产妇,而第三产程时间以及产后出血、侧切撕裂、新生儿呼吸窘迫综合征、Apgar评分等与初产妇相差无异,安全性较好。
Objective To explore the effect of cesarean section on the delivery course of vaginal delivery after the second pregnancy.Methods From January 2017 to December 2020,our hospital selected 80 parturiens who had second pregnancy after cesarean section and met the conditions of natural delivery into the study group,and 40 parturiens who had full term natural delivery were selected as the comparison group during the same period.The two groups did not implement the intervention of artificial rupture of membranes and oxytocin.The duration of labor,rate of lateral incision,incidence of postpartum hemorrhage,tear rate,neonatal asphyxia rate,puerperal infection rate,Apgar score,neonatal umbilical artery pH value and neonatal respiratory distress syndrome were compared between the two groups.Results Compared with the control group,the first stage of labor,the second stage of labor and the total stage of labor were shorter in the study group(P<0.05).There were no significant differences in the lateral incision rate,tear rate,incidence of postpartum hemorrhage and puerperal infection rate between the study group and the control group(P>0.05).There were no significant differences in the incidence of neonatal respiratory distress syndrome,neonatal umbilical artery pH<7.0 and neonatal Apgar score<7 between the study group and the control group(P>0.05).Conclusion After cesarean section,if pregnant women meet the conditions of vaginal delivery,they can choose natural delivery.The time of the first stage of labor,the second stage of labor and the total stage of labor were shorter than that of primiparas,while there was no difference in time of the third stage of labor,postpartum hemorrhage,lateral incision tear,neonatal respiratory distress syndrome,Apgar score between them and primiparas,showing good safety.
