摘要
目的 研究剖宫产后再次妊娠选择阴道分娩(VABC)的影响因素。方法 采用回顾性分析于2016年9月-2017年9月在阳春市人民医院分娩的60例经剖宫产术后阴道试产孕产妇病例做回顾性分析。其中53例阴道分娩成功的将其作为观察组,7例阴道试产失败,将其分为对照组,再对比2组间的相关因素,通过Logistic多因素回归分析出影响VBAC成功率最重要的因素。比较两种分娩结果。结果 产次、入院前宫口有无扩张、产前BMI、不同孕次、有无阴道分娩史、入院前是否宫缩、宫颈Bishop评分是否>3分方面差异有统计学意义(P<0.05)。结论 对剖宫产后再次妊娠患者的分娩方式选择中,在把握适应证以及禁忌证的前提下,可进行阴道分娩,且阴道分娩可降低产后出血、感染的发生率,提高母婴生存质量。
Objective To study influencing factors of vaginal delivery (VABC) for second pregnancy after cesarean section.Methods The paper reviewed and analyzed 60 cases vaginal trial delivery after cesarean section in our hospital from September 2016 to September 2017.Among them,53 cases in observation group had successful vaginal delivery,7 cases in control group with failed vaginal trial delivery.Relevant factors between two groups were compared.Most important factors affecting success rate of VBAC were analyzed by logistic multivariate regression.Delivery outcome was compared.Results There was significant difference in number of births,whether uterine orifice was dilated before admission,prenatal BMI,different times of pregnancy,whether there was vaginal delivery history,whether there was uterine contraction before admission,and whether cervical Bishop scores were>3,(P<0.05).Conclusion During selection of delivery methods for patients with second pregnancy after cesarean section,vaginal delivery can be carried out on condition of grasping indications and contraindications,and vaginal delivery can reduce incidence of postpartum hemorrhage and infection and improve life quality of mothers and infants.
