摘要
冠心病的智能诊疗系统随全球人工智能技术的进步已得到长足的发展,但是由于中医学对人体疾病的认识具有复杂性、动态性、发展性的特点,目前中西医结合的冠心病病证智能诊疗仍未能体现中医思维特色。整体观念与辨证论治是中医思维的基本要素,共性与个性相协调是中医临床思维的一大特点。因此,单纯基于流行病学研究结果的中医诊疗知识模型有悖于中医思维要求。通过分析现有中医诊疗知识模型特点与存在的问题,结合目前常用算法在中医病证结合诊疗模型中的运用,针对冠心病心绞痛提出构建基于中医思维的病证结合诊疗知识模型,即在病证结合模式下引入证候要素概念,以中医思维为指导、标准化为前期基础、病证结合为实现方法、'疾病-证候-证候要素-症状'为诊断顺序、'理-法-方-药'为诊疗路线,从而实现智能化处方用药的目的,促进中西医结合的冠心病病证智能诊疗技术的发展。
The intelligent diagnosis and treatment system for coronary heart disease(CHD)has been greatly developed along with the advancement of global artificial intelligence technology.However,due to the complexity,dynamics and developmental characteristics of traditional Chinese medicine(TCM)for human diseases,the current diagnosis and treatment of CHD with integrated Chinese and Western medicine still fail to reflect the characteristics of TCM thinking.The holistic view and syndrome differentiation are the basic elements of TCM thinking.The coordination of commonality and individuality is a major feature of TCM clinical thinking.The TCM diagnosis and treatment knowledge model based solely on epidemiological research results is contrary to the requirements of TCM thinking.This paper analyzed the characteristics and problems of existing TCM diagnosis and treatment knowledge model,combined with the application of the commonly used algorithms in the TCM diagnosis and treatment model,and put forward the idea of constructing a syndrome-based diagnosis and treatment knowledge model based on TCM thinking for CHD with angina pectoris.That is to introduce the concept of syndrome elements in the combination of disease and syndrome,with the guidance of TCM thinking,standardization as the basis of the previous stage,the combination of disease and syndrome as the realization method,the'disease-syndromes-syndrome elements-symptoms'as the diagnosis order,and the'treatment principles,methods,prescriptions and herbs'as diagnosis and treatment route,in order to achieve the purpose of intelligent prescription,to promote the development of intelligent diagnosis and treatment technology for CHD syndrome with integrated Chinese and western medicine.
