摘要
目的探索中药注射剂专项点评模式,为临床合理用药提供参考。方法调取西安市中心医院2017年7月至2018年6月使用中药注射剂相关病历1 200份,利用帕累托图对不合理用药医嘱进行分析,确定不合理用药主要类型,再用鱼骨图进行根因分析,制定改进措施。结果不合理病历718份(占59.8%),不合理用药医嘱829例次。帕累托图分析显示不合理用药类型中主要因素有适应证不适宜(占44.2%)、用量偏大(占15.3%)、用量偏小(占11.2%)和存在配伍禁忌组液体间未冲管(占9.1%);次要因素是联合用药不适宜;一般因素是疗程偏长、溶媒选择不合理及重复用药。结论通过帕累托图和鱼骨图分析中药注射剂不合理用药类型并干预,形成中药注射剂专项点评模式,提高中药注射剂的合理使用。
Objective To analyze the use of traditional Chinese medicine injections,and to explore the special review mode of traditional Chinese medicine injections,to provide reference for clinical rational drug use.Methods 1 200 relevant medical records of Xi’an Central Hospital from July 2017 to June 2018 using traditional Chinese medicine injections are collected,use Pareto chart to analyze the irrational use of medical orders,determine the main types of unreasonable use,and then the fundamental reasons of the irrational orders are analyzed through fishbone diagram to make improvement plans.Results 718 unreasonable medical records(59.8%),829 unreasonable medical orders.Pareto chart analysis show that the main factors in the irrational drug type are inappropriate indications(44.2%),large dosage(15.3%),small dosage(11.2%),and no injunction between the incompatible liquids(9.1%);secondary factors are unsuitable for combination therapy;the general factors are long course of treatment,unreasonable choice of solvent and repeated drug use.Conclusion Through the analysis of the irrational drug type of Chinese medicine injection with the Pareto chart and fishbone diagram,form special evaluation mode of traditional Chinese medicine injection,and improve the rational use of traditional Chinese medicine injection.
