摘要
目的了解急性冠脉综合征(ACS)患者心理脆弱状况,探讨ACS患者心理复原力在社会支持和心理脆弱间的中介作用。方法横断面研究。连续选取2024年3—12月于北京大学第三医院诊断为ACS的300例住院患者作为调查对象,使用一般资料调查表、社会支持评定量表、心理复原力量表和心理脆弱量表进行问卷调查。根据纳排标准,最终纳入ACS患者286例,中位年龄64.0(55.0,72.0)岁,其中男性209例(73.1%)。采用Spearman相关分析分别探究社会支持、心理复原力及心理脆弱之间的相关性。采用AMOS 26.0软件构建结构方程模型并进行指标中介效应分析。采用Bootstrap法验证指标中介效应。结果ACS患者的社会支持评定量表、心理复原力量表和心理脆弱量表总分依次为(39.8±8.1)分、(64.9±17.0)分和46.5(39.0,55.0)分。将ACS患者的社会支持、心理复原力和心理脆弱总分进行相关性分析,结果显示,心理脆弱与心理复原力(r=-0.159,P<0.001)和社会支持(r=-0.244,P<0.001)呈负相关;心理复原力与社会支持呈正相关(r=0.264,P<0.001)。以心理脆弱为因变量,社会支持为自变量,心理复原力为中介变量,构建结构方程模型,结果显示,社会支持与心理复原力呈正相关(β=0.35,P<0.001),社会支持(β=-0.20,P=0.019)和心理复原力(β=-0.14,P=0.040)均与心理脆弱呈负相关,心理复原力在社会支持与心理脆弱间起中介作用(e=-0.047,95%CI:-0.108~-0.004,P=0.041),占总效应的19.0%。结论社会支持与心理复原力呈正相关,社会支持和心理复原力均与心理脆弱呈负相关,心理复原力在社会支持与心理脆弱间起中介作用。
Objective To understand the status of mental vulnerability in patients with acute coronary syndrome(ACS)and to explore the mediating role of psychological resilience between social support and mental vulnerability in these patients.Methods This was a cross-sectional study.From March to December 2024,300 patients diagnosed with ACS in Peking University Third Hospital were consecutively recruited.Questionnaires were administered,including The General Information Questionnaire,the Social Support Rating Scale,the Connor-Davidson Resilience Scale,and the Mental Vulnerability Questionnaire.According to the inclusion and exclusion criteria,286 patients with ACS were finally included,with a median age of 64.0(55.0,72.0)years,including 209 males(73.1%).Spearman correlation analysis was used to explore the correlations between social support,psychological resilience,and mental vulnerability.AMOS 26.0 software was used to establish a structural equation model and analyze the mediating effect.The Bootstrap method was used to verify the mediating effect.Results The total scores of the social support rating scale,the Connor-Davidson Resilience Scale,and the mental vulnerability questionnaire were 39.8±8.1,64.9±17.0,and 46.5(39.0,55.0),respectively.Spearman correlation analysis was performed on the total scores of social support,psychological resilience and mental vulnerability,and the scores of each dimension in ACS patients.The results showed that the total score of mental vulnerability was negatively correlated with the total score of psychological resilience(r=-0.159,P<0.001)and the total score of social support(r=-0.244,P<0.001).The total score of psychological resilience was positively correlated with the total score of social support(r=0.264,P<0.001).A structural equation model was then constructed with mental vulnerability as the dependent variable,social support as the independent variable,and psychological resilience as the mediating variable.The results showed that social support was positively correlated with psychological resilience(β=0.35,P<0.001).Social support(β=-0.20,P=0.019)and psychological resilience(β=-0.14,P=0.040)were negatively correlated with mental vulnerability.Psychological resilience played a mediating role between social support and mental vulnerability(e=-0.047,95%CI:-0.108--0.004,P=0.041),accounted for 19.0%of the total effect.Conclusions Social support was positively correlated with psychological resilience,while both social support and psychological resilience were negatively correlated with mental vulnerability.Psychological resilience plays a mediating role between social support and mental vulnerability.
