特发性矮小症儿童的25-(OH)D及IGF-1水平检测结果及影响因素分析

摘要 目的探究特发性矮小症(ISS)儿童25羟维生素D[25-(OH)D]、胰岛素样生长因子1(IGF-1)水平及其相关影响因素。方法选取2022年7月—2024年6月广东省梅州市妇幼保健院收治的102例ISS儿童纳入疾病组,另选取同期体检的80例健康儿童纳入健康组,比较两组血清25-(OH)D、IGF-1水平。收集所有患儿临床资料,采用单因素和多因素Logistic回归分析ISS的影响因素。采用受试者工作特征曲线(ROC)分析血清25-(OH)D、IGF-1水平对ISS的预测价值。结果疾病组血清25-(OH)D、IGF-1水平低于健康组(P<0.05);多因素Logistic回归分析显示,骨龄指数、骨龄年龄差、性发育状态、骨钙素、25-(OH)D、IGF-1是ISS的独立危险因素(P<0.05);25-(OH)D、IGF-1诊断ISS的AUC为0.795、0.764。结论25-(OH)D、IGF-1在ISS儿童中低表达,可作为ISS的诊断指标,且ISS的发生与骨龄指数、骨龄年龄差、性发育状态、骨钙素有关。 Objective To investigate the levels of 25-hydroxyvitamin D[25-(OH)D]and insulin-like growth factor 1(IGF-1)in children with idiopathic short stature(ISS)and their related influencing factors.Methods 102 children with ISS admitted to Meizhou Maternal and Child Health Hospital in Guangdong Province from July 2022 to June 2024 were selected as the disease group,and 80 healthy children who underwent physical examinations during the same period were selected as the healthy group.The serum levels of 25-(OH)D and IGF-1 were compared between the two groups.Collect clinical data of all pediatric patients and use univariate and multivariate logistic regression analysis to identify the influencing factors of ISS.The predictive value of serum 25-(OH)D and IGF-1 levels for ISS was analyzed using receiver operating characteristic(ROC)curves.Results The levels of serum 25-(OH)D and IGF-1 in the disease group were lower than those in the healthy group(P<0.05);Multivariate logistic regression analysis showed that bone age index,bone age difference,sexual development status,osteocalcin,25-(OH)D and IGF-1 were independent risk factors for ISS(P<0.05);The AUC of 25-(OH)D,IGF-1 for diagnosing ISS were 0.795,0.764.Conclusion 25-(OH)D and IGF-1 are lowly expressed in children with ISS and can be used as diagnostic indicators for ISS.The occurrence of ISS is related to bone age index,bone age difference,sexual development status,and osteocalcin.
