摘要
目的:观察滋水清肝散结汤对肝郁化火证中枢性性早熟(CPP)女童性发育水平的影响。方法:筛选收集2022年8月至2024年2月于芜湖市中医医院就诊的肝郁化火证CPP女童共60例,根据不同治疗方案,分为对照组与汤药组各30例,对照组行基础治疗,汤药组在对照组基础上给予滋水清肝散结汤治疗,两组患儿治疗疗程均为6个月。回顾性分析两组患儿治疗后的临床疗效、中医证候总积分、身高、骨龄指数、雌二醇(E_(2))、黄体生成素(LH)、卵泡雌激素(FSH)、子宫卵巢容积与最大卵泡直径的变化。结果:治疗后汤药组患儿总有效率为86.67%,显著优于对照组患儿总有效率的63.33%(P<0.05)。两组患儿的中医证候积分、骨龄指数、性激素及性发育水平较治疗前均有下降(P<0.05),身高较治疗前增长(P<0.05)。治疗后两组患儿组间比较,身高、FSH、E_(2)及卵巢容积差异无统计学意义(P>0.05),汤药组患儿中医证候积分、骨龄指数、LH、子宫容积及最大卵泡直径下降幅度较对照组患儿更为显著(P<0.05)。结论:滋水清肝散结汤治疗肝郁化火证中枢性性早熟女童临床疗效较为显著,能改善性早熟女童急躁易怒、乳房胀痛、面赤潮热等肝郁化火症状,延缓骨龄生长,降低性激素及性发育水平,具有临床推广意义。
Objective:To investigate the effects of Nourishing water,clearing the liver and dispersing stasis Soup on the sexual development of girls with central precocious puberty of liver stagnation and fire syndrome.Methods:Sixty cases of central precocious puberty girls with liver stagnation and fire syndrome screened in the Wuhu Hospital of Chinese Medicine from August 2022 to February 2024 were selected and collected,and according to different treatment plans,30 girls were separated into the control group and the others 30 girls were in the decoction group.The control group received the basic treatment,while the decoction group was offered nourishing water,clearing the liver and dispersing stasis decoction,based on the control group.Both groups conducted the research over six months.A retrospective analysis was performed on the changes in clinical efficacy,total score of TCM syndromes,height,bone age index,LH,FSH,E_(2),uterine volume,ovarian volume,and the largest follicle diameter of the two groups were investigated after treatment.Results:After treatment,the whole valid rate of the decoction group was 86.67%,obviously higher than the control group,which was 63.33%.(P<0.05).Children in both groups showed a decrease in score of TCM syndromes,bone age index,sex hormone levels,and sexual development level(P<0.05),as well as a growth in height compared to pre-treatment values(P<0.05).After treatment,the differences in growth,FSH,E_(2) and ovarian volume were not statistically significant(P>0.05),while the score of TCM syndromes,bone age index,LH,uterine volume and the largest diameter of the follicles was smaller in the decoction group than in the other group(P<0.05).Conclusion:Nourishing water,clearing the liver and dispersing stasis decoction has a more significant clinical effect on signs of liver stagnation and fire syndrome in early adolescent girls,and can reduce symptoms of liver stagnation and fire syndrome,such as impatience and irritability,swelling and chest pain,facial flushing and hot flashes in early adolescent girls.It slows the growth of bone age,reduces the level of sex hormones and sexual development,which is very important to promote clinical significance.
