摘要
目的:探讨基于加速康复外科(ERAS)理念的早期系统康复干预在严重肺挫伤合并多发肋骨骨折患者中的应用效果。方法:选取郑州大学附属郑州中心医院ICU收治的96例严重肺挫伤合并多发肋骨骨折患者作为研究对象,按照入院时间将2023年2月—8月入院的患者设为对照组(48例,接受常规干预),将2023年9月—2024年4月入院的患者设为试验组(48例,在接受标准干预的基础上接受基于ERAS理念的早期系统康复干预)。两组均干预6个月,观察两组疼痛程度、持续时间及恢复情况。结果:干预后,试验组疼痛评分低于对照组,疼痛持续时间短于对照组(P均<0.05)。干预后,试验组引流管留置时间和机械通气时间均短于对照组,肺部感染、肺不张、深静脉血栓形成以及呼吸衰竭的发生率均低于对照组(P均<0.05)。两组ICU停留时间和ICU住院费用对比,差异无统计学意义(P > 0.05)。结论:基于ERAS理念的早期系统康复干预在严重肺挫伤合并多发肋骨骨折患者中的应用效果确切。
Objective:To explore the application effect of early systematic rehabilitation intervention based on the enhanced recovery after surgery(ERAS)concept in patients with severe pulmonary contusion and multiple rib fractures.Methods:A total of 96 patients with severe pulmonary contusion and multiple rib fractures admitted to the ICU of Zhengzhou Central Hospital Affiliated to Zhengzhou University were selected as the study subjects.According to the admission time,patients admitted from February to August 2023 were assigned to the control group(48 cases,receiving conventional intervention),while those admitted from September 2023 to April 2024 were assigned to the experimental group(48 cases,receiving early systematic rehabilitation intervention based on the ERAS concept in addition to standard intervention).Both groups were intervened for 6 months,and the pain level,duration,and recovery were observed.Results:After the intervention,the experimental group had lower pain scores and shorter pain duration than the control group(P<0.05).Additionally,the experimental group exhibited shorter durations of drainage tube retention and mechanical ventilation,as well as lower incidences of pulmonary infection,atelectasis,deep vein thrombosis,and respiratory failure compared to the control group(P<0.05).There was no significant difference in ICU length of stay or hospitalization costs between the two groups(P>0.05).Conclusion:Early systematic rehabilitation intervention based on the ERAS concept demonstrates definite efficacy in patients with severe pulmonary contusion and multiple rib fractures.
