摘要
目的 了解医院患者送检病原学标本检出病原菌种类及耐药性情况,为临床合理应用抗菌药物提供参考。方法 通过回顾性分析方法,对某三级医养结合型医院患者送检病原学标本病原菌及其耐药性检测结果进行统计分析。结果 纳入分析的病原菌共计24 014株,其中革兰阴性菌占79.1%,革兰阳性菌占15.6%,真菌占5.3%。排名前5位为铜绿假单胞菌、大肠埃希菌、肺炎克雷伯菌、金黄色葡萄球菌和鲍曼不动杆菌,构成比依次为25.2%、17.3%、10.0%、7.5%和5.7%。临床分离大肠埃希菌对环丙沙星、头孢唑林和头孢曲松的耐药率>70%;鲍曼不动杆菌对头孢类和环丙沙星等耐药率均>70%。结论 该医院临床送检病原学标本检出以革兰阴性菌为主,对临床常用抗菌药物普遍耐药,应加强抗菌药物的合理使用和管理。
Objective To understand the types and drug resistance of pathogenic bacteria detected in pathogenic specimens submitted by hospital patients,and to provide reference for the rational use of antibiotics in clinical practice.Methods Using retrospective analysis,the results of pathogenic bacteria and drug resistance testing of the pathogen specimens submitted by patients from a tertiary medical and elderly care integrated hospital were statistically analyzed.Results A total of 24014 strains were isolated,with Gram negative bacteria accounting for 79.1%,Gram positive bacteria accounting for 15.6%,and fungi accounting for 5.3%.The top five were Pseudomonas aeruginosa,Escherichia coli,Klebsiella pneumoniae,Staphylococcus aureus,and Acinetobacter baumannii,with composition ratios of 25.2%,17.3%,10.0%,7.5%,and 5.7%,respectively.The resistance rates of clinically isolated Escherichia coli to ciprofloxacin,cefazolin,and ceftriaxone were>70%;The resistance rates of Acinetobacter baumanni to cephalosporins,ciprofloxacin and other antibiotics were all>70%.Conclusion Gram negative bacteria are the main pathogens detected in clinical specimens submitted for examination in this hospital,and they are generally resistant to commonly used antibiotics in clinical practice.Therefore,it is necessary to strengthen the rational use and management of antibiotics.
