摘要
胃癌前病变是消化系领域中具有较高癌变风险的病理改变,及时防治能够延缓甚至阻断胃癌的发生。中医药以辨证论治为基础,具有用药灵活、疗效显著、副作用小的独特优势,在胃癌前病变的治疗领域受到了广泛关注。谢晶日教授从事治疗胃癌前病变多年,有着丰富的临床经验,认为“脾胃虚弱,气机难畅”是其发病根本,“气滞血瘀”是疾病发生发展的重要病机基础,“病邪胶结,聚成瘀毒”为其癌变的关键;因此根据胃癌前病变的病机特点,以“益气化瘀”为治疗原则,兼以行气活血、顾护胃阴,随证加减,真正达到药证结合,标本兼治的效果。文章基于“益气化瘀法”介绍谢晶日教授治疗本病的临证经验,以期更好地指导临床诊治。
Gastric precancerous lesion(GPL)is a pathological change with high cancer risk in the field of digestive system,and timely prevention and treatment can delay or even block the occurrence of gastric cancer.Traditional Chinese medicine,which is based on evidence-based treatment and has the unique advantages of flexible medication,significant therapeutic effects,and few side effects,has received widespread attention in the field of GPL treatment.Professor Xie Jingri has been engaged in the treatment of GPL for many years and has rich clinical experience.He thinks that“weakness of the spleen and stomach,difficult to smooth the qi”is the root of its pathogenesis,“stagnation of qi and blood stasis”is an important stage of the disease development,and the key to cancerous transformation is“the accumulation of stasis and poison”.Therefore,according to the characteristics of GPL s pathogenesis,“benefiting qi and removing blood stasis”is the principle of treatment,along with promoting qi and blood circulation,protecting stomach yin,and the flexible use of clinical evidence,the curative effect of combining medicine and syndrome and the treatment of both symptoms and root causes was truly achieved.The article introduces the clinical experience of Professor Xie Jingri in treating this disease based on the method of“benefiting qi and removing blood stasis”,in order to better guide the clinical diagnosis and treatment.
