摘要
中风作为临床高发脑系疾病,是中国成人致死、致残的首要病因,严重危害公众健康。中医诊疗虽具挑战,但基于独特理论体系和显著效果优势,已成为中医临床优势病种之一。本文立足“扶阳法”理论,系统阐明从五脏辨治中风的学术观点,提出中风根源是五脏阳气失司:心阳不振致瘀阻神衰,肾阳亏虚致髓亏气化失司,肝阳郁亢致气血逆乱,脾阳困遏致痰浊痹阻,肺阳不足致卫外失固。针对核心病机,创立“温通-温固-调阳-助阳-补散”五法协同的扶阳治则体系:温通心阳以振神明,培元固肾以充髓海助气化,疏肝调阳以平亢逆,运脾助阳以复升降,温肺补气以固本元。旨在通过调节脏腑阳气,重建“阳主阴从”秩序,为中风辨治提供系统思路。
Apoplexy as a highly prevalent neurological disease in clinical practice,is the leading cause of death and disability in Chinese adults,posing a serious threat to public health.Although traditional Chinese medicine diagnosis and treatment is challenging,based on unique theoretical system and significant advantages in effectiveness,it has become one of the advantageous clinical diseases in traditional Chinese medicine.This article is based on the theory of“yang-nourishing method”,and systematically elaborates on academic viewpoint of syndrome differentiation and treatment of apoplexy from five zang viscera,proposes that the root cause of apoplexy is the loss of yang qi in five zang viscera:heart yang deficiency leads to blood stasis and spiritual decline,deficiency of kidney yang leads to marrow deficiency and loss of qi,excessive liver yang depression leads to disorder of qi and blood flow,spleen yang stagnation leads to phlegm turbidity and obstruction,insufficient lung yang leads to external instability.Aiming at core pathogenesis,establish synergistic system of the five methods of“warming and promoting circulation-warming and solidifying-regulating yang-assisting yang-supplementing powder”for yang-nourishing method:warming and dredging heart yang to invigorate spirit,strengthening foundation and tonifying kidney to enrich the sea of marrow and assist qi transformation,soothing liver and regulating yang to subdue hyperactivity,promoting spleen and assisting yang to restore the ascending and descending functions,and warming lung and tonifying qi to consolidate the foundation.Intended to provide a systematic approach for syndrome differentiation and treatment of apoplexy by regulating yang qi of zang fu viscera,and rebuilding the order of“yang dominates and yin follows”.
