摘要
中风后遗症是临床常见的心脑血管疾病,其发病率不断升高,造成了严重的社会经济负担。中风后遗症患者由于半身不遂、言语不利等原因,日常生活、工作存在诸多不便,生活质量低,且伴随焦虑、抑郁等心理问题。中医将中风后遗症归属于“中风”“偏枯”“偏风”等范畴,一般认为气虚血瘀为其基本病机,以益气活血化瘀作为治疗常法。刘如秀教授根据古今医家认识及多年临床经验认为中风后遗症虽以气虚血瘀为主要病机,但仍有许多患者存在风、火、痰等邪气交杂的病理过程。因此,总结中风的发病需重肝阳之机,若肝阳上亢,当平肝熄风,潜阳降逆;若肝火炽盛,宜清肝泻火,疏肝养阴;若水不涵木,需育阴潜阳,滋水涵木。同时,通阳祛邪合而施治,若元气亏虚,则益气通阳;若痰浊壅盛,予以消痰导滞;若瘀血生风,治以养血化瘀搜风。如此,泄阳与补阳并而施治,正气存而邪火祛,人体阴阳平衡,则疾可疗,病自安。
Stroke sequela is a common cardiovascular and cerebrovascular disease.Its incidence has been increasing,causing serious social and economic burdens.Stroke sequelae patients have many inconveniences in daily life and work,low quality of life,anxiety,depression,and other psychological problems due to hemiplegia,adverse speech and other reasons.Traditional Chinese medicine attributes the sequelae of stroke to the categories of“stroke”and“hemiplegia”.It is generally believed that Qi deficiency and blood stasis are the basic pathogenesis,and supplementing Qi,activating blood circulation and removing blood stasis are the common methods of treatment.According to the understanding of ancient and modern doctors and many years of clinical experience,Professor LIU Ruxiu believes that although Qi deficiency and blood stasis are the main pathogenesis of stroke sequelae,there are still many patients with the pathological process of wind,fire,phlegm and other evil Qi.To sum up,the pathogenesis of stroke needs to pay attention to liver Yang.If the liver Yang is hyperactive,the treatment should calm liver and extinguish wind,and subdue Yang and reduce the inverse of Qi.If the liver fire is blazing,it is appropriate to clear liver and purge fire,soothe liver and nourish Yin.If water does not nourish the wood,the treatment needs to cultivate Yin and subdue Yang,and nourish water and wood.At the same time,the combination of activating Yang and eliminating pathogenic factors can be used for the treatment.If the original Qi is deficient,it will replenish Qi and activate Yang.If there is phlegm stagnation,the treatment should be eliminating phlegm and stagnation.If there is wind due to the blood stasis,the treatment should be nourishing blood and removing blood stasis.In this way,the combination of dredging Yang and tonifying Yang can make the vital Qi strong and clear the evil fire and Yin and Yang of the human body can be balanced,and so the disease can be cured and relieved.
