摘要
目的无症状性脑梗死(Silent brain infarction,SBI)是易被忽视的脑卒中类型,在老年人群中发病率高,本文探讨老年无症状性脑梗死危险因素。方法收集老年患者的160例临床资料进行回顾性病例对照研究,是否存在脑梗死病灶分为SBI组及非SBI组。结果SBI组在合并高血压病史高于非SBI组,高密度脂蛋白低于非SBI组,多因素logistic回归模型结果显示,高血压与SBI发生有相关性(OR=3.506,OR95%CI:1.40-8.78,P<0.05)。高密度脂蛋白与老年SBI发生有相关性,(OR=0.186,OR95%CI:0.059-00.58,P<0.05)。结论高血压为SBI发生的促进因素,高密度脂蛋白为SBI发生的阻碍因素。
Objective Silent brain infarction(SBI)is easily neglected stroke type,although no clinical symptoms,but high incidence,potential harm,there may be cognitive impairment,gait abnormalities,hidden functional defects,high incidence in the elderly population,but certainty research evidence is insufficient,this paper discusses the risk factors of elderly asymptomatic cerebral infarction,aims to provide reference for prevention strategy.Methods The clinical data of 160 elderly patients without signs of neurological defect were collected for a retrospective case-control study and divided into SBI and non-SBI groups.Results SBI group had higher history of hypertension than non-SBI group,and high-density lipoprotein was lower than non-SBI group,the difference was statistically significant(P<0.05),and the multivariate logistic regression model was constructed for SBI.The Omnibus test and hosmer-lemesho test suggest that the model is overall meaningful and fits well.The results showed that hypertension was associated with the occurrence of SBI OR=3.506,OR95%CI:1.40-8.78,P<0.05),and hypertension was a contributing factor to the occurrence of SBI.HDL was correlated with the occurrence of SBI in old age,OR=0.186,OR95%CI:0.059-00.58,P<0.05).HDL was a hindering factor for the occurrence of SBI in old age.Conclusions Hypertension is a promoting factor for the development of Silent brain infarction and HDL is a hindering factor for the development of SBI.
