基于机器学习的糖尿病肾脏病预测模型的研究及应用进展

摘要 糖尿病肾脏病(diabetic kidney disease,DKD)发病率逐年上升,已成为终末期肾病(end-stage renal disease,ESRD)的首要病因。构建科学的DKD预测模型有利于尽早、准确、便利地筛查DKD高风险人群,以及时有效地采取防治措施、减轻医疗负担。随着医疗大数据和人工智能技术的进步,不少国内外学者基于机器学习(machine learning,ML)所构建的DKD风险预测模型取得了良好的预测性能。选择适宜的ML算法进行DKD预测模型的构建可提升模型的性能,进而提高DKD高风险人群的筛查效率和精确率,节约治疗成本,更好地辅助临床医生采取个体化的干预措施。文章通过对目前ML算法在DKD风险预测模型构建中的应用进行综述,旨在为临床构建适宜的DKD风险预测模型提供依据,为《中国糖尿病肾脏病防治指南》后期的修订提供参考。 The incidence of diabetic kidney disease(DKD)is increasing year by year,which has become the primary cause of end-stage renal disease.Constructing a scientific DKD prediction model is conducive to screening the high-risk population of diabetic kidney disease as soon as possible,accurately and conveniently,and taking timely and effective preventive measures to reduce the medical burden.With the progress of medical big data and artificial intelligence technology,many domestic and foreign scholars have built a DKD risk prediction model based on machine learning(ML)and achieved good prediction performance.Choosing the appropriate ML algorithm to construct the DKD prediction model can improve the performance of the model,and then improve the screening efficiency and accuracy of DKD high-risk population,save the treatment cost,and better assist clinicians to take individualized intervention measures.In this paper,the application of ML algorithm in the construction of DKD risk prediction model is reviewed,aiming at providing a basis for the clinical construction of a suitable DKD risk prediction model,and provides a reference for the later revision of relevant clinical guidelines.
