摘要
五苓散在《伤寒杂病论》和《金匮要略》中均有记载,常用于治疗蓄水证、水逆证等,方中茯苓、猪苓、泽泻从不同方面行利湿之功,佐以白术健脾运湿,桂枝助阳化气,各药相辅相成共奏利水渗湿之功。实验研究表明,五苓散能够通过调控水通道蛋白AQP1-5来调控水液代谢,通过调节RAAS系统、AMPK、Gas6/Axl、IκB/NF-κB、Keap1/Nrf2、TGF-β/Smad、Wnt/β-catenin信号通路来发挥保护肾脏的作用,调节mURAT1和mGLUT9、mOAT1的表达调控糖脂代谢,通过调控RAAS系统、调节ANP分泌发挥降低高血压的作用。临床应用表明,五苓散广泛用于治疗肝肾疾病、心力衰竭、各种水肿、糖脂代谢紊乱等疾病。文献研究表明,五苓散临床应用类文献在相关文献报道中占比较大,但是目前对五苓散的现代机理研究较少,因此对五苓散的临床应用及现代机理研究进行整理分析,可为加强五苓散机制研究和临床合理应用提供思路。
Wuling Powder was recorded in Treatise on Febrile Diseases and Synopsis of Golden Chamber.It is often used to treat water retention syndrome and water reverse syndrome.In the prescription,poria,poria,Zexia from different aspects to perform the work of dampness,with white art to strengthen spleen dampness,laurel branch to help Yang Qi,each medicine complement each other to play the work of water oozing dampness.Experimental studies have shown that Wuling Powder can regulate water metabolism by regulating aquaporin AQP1-5,and protect kidney by regulating RAAS system,AMPK,Gas6/Axl,IκB/NF-κB,Keap1/Nrf2,TGF-β/Smad,Wnt/β-catenin signaling pathways.Regulating the expression of mURAT1,mGLUT9 and mOAT1 regulates glucose and lipid metabolism,and plays a role in reducing hypertension by regulating RAAS system and regulating ANP secretion.Clinical application shows that Wuling Powder is widely used in the treatment of liver and kidney diseases,heart failure,various edema,disorders of glucose and lipid metabolism.Literatures on the clinical application of Wuling Powder account for a large proportion in the relevant literature reports,but there are few studies on the modern mechanism of Wuling Powder at present.Therefore,the clinical application and modern mechanism research of Wuling Powder are sorted out and analyzed in order to provide ideas for further mechanism research and clinical rational application of Wuling Powder.
