天丹通络片联合丁苯酞治疗对急性脑梗死患者临床疗效、血液流变学指标及神经功能的影响

摘要目的探讨天丹通络片联合丁苯酞治疗对急性脑梗死患者临床疗效、血液流变学指标及神经功能的影响。方法回顾性分析周口东新医院2021年4月—2024年2月收治的急性脑梗死患者91例,按照治疗方案分为对照组(丁苯酞治疗,n=45)、试验组(天丹通络片联合丁苯酞治疗,n=46)。比较两组临床疗效、血液流变学指标[全血低切黏度、全血高切黏度、血浆黏度、纤维蛋白原]、神经功能[美国国立卫生研究院卒中量表(National Institutes of Health stroke scale,NIHSS)]及不良反应发生率。结果试验组(93.48%)的临床总有效率显著高于对照组(75.56%)(P<0.05)。两组治疗后的全血低切黏度、全血高切黏度、血浆黏度及纤维蛋白原均降低,且试验组更低(P<0.05)。两组治疗后的NIHSS评分均下降,且试验组更低(P<0.05)。试验组的不良反应发生率为10.87%,对照组的不良反应发生率为6.67%,两组比较差异无统计学意义(P>0.05)。结论天丹通络片联合丁苯酞治疗可有效改善急性脑梗死患者血液流变学指标水平,促进神经功能的恢复,且临床疗效和安全性较好。 Objective To investigate the effects of Tiantan Tongluo tablets combined with Butylphthalide treatment on the clinical efficacy,hemorheological indexes and neurological function of patients with acute cerebral infarction.Methods A retrospective analysis was conducted on of 91 patients with acute cerebral infarction admitted to Zhoukou Dongxin Hospital from April 2021 to February 2024.According to the treatment plan,they were divided into control group(Butylphthalide treatment,n=45)and experimental group(Tiantan Tongluo tablets combined with Butylphthalide treatment,n=46).Clinical efficacy,hemorheological indexes[whole blood low shear viscosity,whole blood high shear viscosity,plasma viscosity,fibrinogen],neurological function[National Institutes of Health stroke scale(NIHSS)]and incidence of adverse effects were compared between the two groups.Results The total clinical effectiveness rate of the experimental group(93.48%)was significantly higher than that of the control group(75.56%)(P<0.05).Whole blood low shear viscosity,whole blood high shear viscosity,plasma viscosity and fibrinogen were reduced in both groups after treatment,and were lower in the experimental group(P<0.05).NIHSS scores decreased in both groups after treatment and were lower in the experimental group(P<0.05).The incidence of adverse reactions was 10.87%in experimental group and 6.67%in control group,and there was no statistical significant difference between the two groups(P>0.05).Conclusion Tiantan Tongluo tablet combined with butylphthalide treatment can effectively improve the level of hemorheological indexes in patients with acute cerebral infarction,promote the recovery of neurological function,and have good clinical efficacy and safety.
