摘要
目的探讨滋肾健脾养宫汤联合来曲唑治疗肾虚血瘀证多囊卵巢综合征(PCOS)的临床疗效,以及对患者肠-脑轴和血液流变学指标的影响。方法选取河北省沧州市人民医院和河北省沧州市中心医院2021年6月至2022年12月收治的肾虚血瘀证PCOS患者110例,按治疗方案的不同分为对照组(52例)和观察组(58例)。两组患者均予来曲唑,观察组患者加用滋肾健脾养宫汤。两组患者均连续治疗3个月经周期。结果观察组总有效率为91.38%,显著高于对照组的73.08%(P<0.05)。治疗后,两组患者多毛和痤疮评分、左侧和右侧卵巢体积均显著降低(P<0.05),且观察组均显著低于对照组(P<0.05);两组患者黄体生成素(LH)、睾酮、LH/促卵泡激素水平均显著降低(P<0.05),雌二醇水平均显著升高(P<0.05),且观察组均显著优于对照组(P<0.05);两组患者高切全血黏度、低切全血黏度、红细胞沉降率、血细胞比容均显著低于治疗前(P<0.05),且观察组均显著低于对照组(P<0.05);两组患者胃饥饿素、胰高血糖素样肽-1、酪酪肽、短链脂肪酸水平均显著升高(P<0.05),且观察组均显著高于对照组(P<0.05);两组患者中医证候积分均显著降低(P<0.05),且观察组显著低于对照组(P<0.05)。观察组和对照组患者不良反应发生率相当(13.79%比13.46%,P>0.05)。结论滋肾健脾养宫汤联合来曲唑治疗肾虚血瘀证PCOS的临床疗效良好,能有效调节患者的肠-脑轴、内分泌代谢和性激素水平,改善月经情况和血液流变学指标,且安全性良好。
Objective To investigate the clinical efficacy of Zishen Jianpi Yanggong Decoction combined with letrozole in the treatment of polycystic ovary syndrome(PCOS)with kidney deficiency and blood stasis syndrome,and its effect on the gut-brain axis and hemorheological indexes.Methods A total of 110 PCOS patients with kidney deficiency and blood stasis syndrome admitted to the Cangzhou People′s Hospital and Cangzhou Central Hospital from June 2021 to December 2022 were selected and divided into the control group(52 cases)and the observation group(58 cases)based on treatment regimens.The patients in the two groups received letrozole,while the patients in the observation group additionally received Zishen Jianpi Yanggong Decoction.Both groups were continuously treated for three menstrual cycles.Results The total effective rate in the observation group was 91.38%,which was significantly higher than 73.08%in the control group(P<0.05).After treatment,the hirsutism and acne scores,left and right ovarian volumes in the two groups significantly decreased(P<0.05),and those in the observation group were significantly lower than those in the control group(P<0.05);the levels of luteinizing hormone(LH),testosterone,and LH/follicle-stimulating hormone(FSH)in the two groups significantly decreased,while the level of estradiol in the two groups significantly increased(P<0.05),and those in the observation group were significantly better than those in the control group(P<0.05);the high-and low-shear whole blood viscosity,erythrocyte sedimentation rate,and hematocrit in the two groups significantly decreased(P<0.05),and those in the observation group were significantly lower than those in the control group(P<0.05);the levels of ghrelin,glucagon-like peptide-1(GLP-1),peptide YY(PYY),and short-chain fatty acid in the two groups significantly increased(P<0.05),and those in the observation group were significantly higher than those in the control group(P<0.05);the traditional Chinese Medicine(TCM)syndrome score in the two groups significantly decreased(P<0.05),and that in the observation group was significantly lower than that in the control group(P<0.05).The incidence of adverse reactions was comparable between the observation group and the control group(13.79%vs.13.46%,P>0.05).Conclusion Zishen Jianpi Yanggong Decoction combined with letrozole in the treatment of PCOS with kidney deficiency and blood stasis syndrome has good efficacy and safety,which can effectively regulate the gut-brain axis,endocrine metabolism,and sex hormone levels,and improve menstrual conditions and hemorheological indexes.
