摘要
心肌纤维化(myocardial fibrosis,MF)通常指的是心肌组织中细胞外基质成分的异常累积,特别是胶原蛋白的过量堆积以及其在组织内的比例和排列状态的失衡。这一病理状态表现为心肌细胞间质空间的填充增加,影响了心肌组织的正常结构和功能,是几乎所有形式心脏病的常见病理结果,也是心力衰竭的主要风险因素。Wnt/β连环蛋白(β-catenin)信号通路是胚胎发育和成体组织稳态再生所需要的,Wnt/β-catenin信号通路的活化对心脏成纤维细胞的形成具有促进作用,这会进一步诱导心肌纤维化和心肌细胞的肥大现象。心肌纤维化在中医中并没有直接对应的病名,根据临床表现和病理特点,可以将其归入中医的“心悸”“胸痹”“真心痛”“心衰”等疾病范畴中进行辨证施治,该文搜集了近年来中医药基于Wnt/β-catenin通路治疗心肌纤维化研究的相关文献,通过梳理和归纳,以期为心肌纤维化的治疗方法带来新的视角和思路。
myocardial fibrosis(MF)is commonly referred to as an abnormal accumulation of extracellular matrix components in myocardial tissue,particularly an excessive accumulation of collagen and an imbalance in its proportion and arrangement in the tissue.This pathological state,manifested by increased filling of the interstitial space of cardiomyocytes,affects the normal structure and function of myocardial tissue and is a common pathological outcome of almost all forms of heart disease and a major risk factor for heart failure.The Wnt/β-catenin signaling pathway is required for embryonic development and adult tissue homeostasis regeneration,and activation of the Wnt/β-catenin signaling pathway promotes the formation of cardiac fibroblasts,which further induces myocardial fibrosis and cardiomyocyte hypertrophy.Myocardial fibrosis does not have a direct corresponding name in traditional Chinese medicine.According to the clinical manifestations and pathological characteristics,it can be classified into the categories of"heart palpitations","chest numbness","true pain","heart failure"and other diseases of traditional Chinese medicine for syndrome differentiation and treatment.This paper collected the relevant literature on the treatment of myocardial fibrosis based on Wnt/β-catenin pathway in recent years.Through combing and induction,it is expected to bring new perspectives and ideas for the treatment of myocardial fibrosis.
