摘要
目的 探讨双能X线骨密度测量(dual-energy X-ray absorptiometry,DXA)联合胸腹部CT扫描检查骨密度筛查骨质疏松症(OP)的效果。方法 收集2021—2023年门诊就诊的116例疑似OP患者,均行腰椎正位DXA及CT扫描检查,2种检查方式间隔不超过1周。比较单独DXA、DXA联合胸腹部CT扫描的OP检出率。结果 116例患者中,男64例,年龄(53.5±13.9)岁;女52例,年龄(48.3±12.8)岁。DXA联合胸腹部CT扫描检出OP18例(男性8例,女性10例),占15.52%;DXA检出OP 9例(男性3例,女性6例),占7.76%。DXA联合胸腹部CT扫描OP检出率高于单独DXA(χ^(2)=15.323,P <0.001),2种检查方式的女性OP检出率均高于男性(P <0.001)。结论 性别是OP筛查的主要影响因素。DXA联合胸腹部CT扫描可提高OP的检出率,DXA对骨密度的测量可能存在局限性。
Objective To investigate the effectiveness of combined dual-energy X-ray absorptiometry(DXA)and thoracoabdominal CT scanning in bone mineral density assessment for osteoporosis(OP)screening.Methods A total of 116 outpatients with suspected OP from 2021 to 2023 were enrolled,all of whom underwent posteroanterior lumbar DXA and CT scanning within a one-week interval.The OP detection rates of DXA alone versus DXA combined with thoracoabdominal CT scanning were compared.Results The cohort consisted of 64 males(53.5±13.9 years)and 52 females(48.3±12.8 years).Combined DXA and CT detected OP in 18 cases(8 males,10 females;15.52%),while DXA alone identified 9 cases(3 males,6 females;7.76%).The combined approach demonstrated a significantly higher OP detection rate compared to DXA alone(χ^(2)=15.323,P<0.001).Females exhibited higher OP detection rates than males across both modalities(P<0.001).Conclusion Gender is a significant influencing factor in osteoporosis screening.The combination of DXA with thoracoabdominal CT scanning may enhance OP detection rate,while DXA measurements might exhibit certain limitations in bone mineral density assessment.
