摘要
目的探讨风险预警干预策略在预防新生儿经外周静脉穿刺中心静脉置管(PICC)患儿非计划拔管中的应用效果。方法选取郑州大学第一附属医院2022年3—10月接受PICC置管的120例新生儿为研究对象,根据随机数字表法分为Ⅰ组(61例),Ⅱ组(59例)。Ⅰ组接受风险预警干预,Ⅱ组接受优质护理措施。分别比较两组新生儿穿刺点肿胀及渗血程度、并发症发生率、非计划拔管率和家属心理状态。结果干预后Ⅰ组穿刺点肿胀及渗血程度均低于Ⅱ组(P<0.05);Ⅰ组并发症(静脉炎、导管堵塞、导管脱出、感染)总发生率和非计划拔管率低于Ⅱ组(P<0.05);Ⅰ组患儿家属焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于Ⅱ组(P<0.05)。结论风险预警干预策略在预防新生儿PICC置管患儿非计划拔管中能有效降低并发症发生率和非计划拔管率,减轻家属焦虑、抑郁程度。
Objective To explore the application effect of risk early warning intervention strategy on the prevention of unplanned extubation in neonates with peripherally inserted central venous catheter(PICC).Methods A total of 120 neonates undergoing PICC catheterization from March to October 2022 in the First Affiliated Hospital of Zhengzhou University were selected as the study subjects and were divided into groupⅠ(61 cases)and groupⅡ(59 cases)according to the random number table method.GroupⅠreceived risk early warning intervention,and groupⅡwas given high-quality nursing measure.The swelling degree and bleeding degree at the puncture point,incidence rates of complications,unplanned extubation rate and psychological status of family members were compared between the two groups.Results After intervention,the swelling degree and bleeding degree at the puncture point in groupⅠwere milder than those in groupⅡ(P<0.05),the total incidence rate of complications(phlebitis,catheter obstruction,catheter extrusion,infection)and unplanned extubation rate were lower in groupⅠthan those in groupⅡ,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)of family members in groupⅠwere significantly lower compared with those in groupⅡ(P<0.05).Conclusion Risk early warning intervention strategy can effectively reduce the incidence rates of complications and unplanned extubation rate,and relieve the anxiety and depression in neonates with PICC catheterization.
