摘要
目的分析在常规治疗基础上应用通心络和阿托伐他汀钙片治疗老年冠心病患者的临床疗效。方法按照纳入和排除标准,选取社旗县人民医院心血管内科2022年1月至2023年12月期间收治的80例老年冠心病患者,随机分为对照组(40例)和研究组(40例)。在常规治疗基础上,对照组接受阿托伐他汀钙片治疗,研究组患者接受通心络胶囊联合阿托伐他汀钙片,两组患者持续治疗3个月。统计并对比两组患者的基线健康情况,包括病程、病变血管和病变血管数、美国纽约心脏病协会(NYHA)心功能分级、是否合并心绞痛、合并基础疾病情况、血压和血脂相关指标水平。同时评估并对比两组患者入院后及治疗3个月后心血管指标和血液流变学指标的变化。统计对比两组患者不良反应发生率差异。结果两组患者的基线资料比较,差异无统计学意义(P>0.05)。入院后两组患者的心功能指标及血液流变学指标比较,差异无统计学意义(P>0.05)。经3个月治疗后,两组患者的收缩压、舒张压、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、全血黏度(WBV)、血小板黏附率(PAR)、血浆黏度(PV)以及纤维蛋白原(FIB)均降低,高密度脂蛋白胆固醇(HDL-C)与左心室射血分数(LVEF)均增高,差异均有统计学意义(P<0.05);组间对比研究组患者的收缩压、舒张压、LDL-C、TC、TG、LVEDD、LVESD、WBV、PAR、PV以及FIB水平均低于对照组,HDL-C和LVEF水平均高于对照组,差异均有统计学意义(P<0.05)。两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论在常规治疗上应用通心络胶囊联合阿托伐他汀钙片相较于单纯应用阿托伐他汀钙片更有助于改善老年冠心病患者的血压和血脂水平,改善患者的心功能和血液流变学指标,临床疗效更理想。
Objective To analyze the clinical efficacy of Tongxinluo capsules combined with atorvastatin calcium tablets in the treatment of elderly patients with coronary heart disease on the basis of conventional therapy.Methods According to the inclusion and exclusion criteria,80 elderly patients with coronary heart disease who were admitted to the Department of Cardiovascular Medicine at Sheqi County People's Hospital from January 2022 to December 2023 were selected and randomly divided into a control group(40 cases)and a study group(40 cases).On top of conventional treatment,the control group received atorvastatin calcium tablets,while the study group received Tongxinluo capsules in conjunction with atorvastatin calcium tablets.Both groups were treated for 3 months.Baseline health conditions were compared,including the duration of disease,affected vessels,number of affected vessels,New York Heart Association(NYHA)functional classification,presence of angina,underlying diseases,blood pressure,and lipid-related indicators.Changes in cardiovascular and hemorheological parameters were assessed and compared between the two groups after admission and at the end of 3 months of treatment.The incidence of adverse reactions was also statistically analyzed.Results There were no statistically significant differences in baseline characteristics between the two groups(P>0.05).No statistically significant differences were noted in heart function and hemorheological parameters of both groups after admission(P>0.05).After 3 months of treatment,both groups exhibited reductions in systolic blood pressure,diastolic blood pressure,low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglycerides(TG),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),whole blood viscosity(WBV),platelet adhesion rate(PAR),plasma viscosity(PV),and fibrinogen(FIB),accompanied by increases in high-density lipoprotein cholesterol(HDL-C)and left ventricular ejection fraction(LVEF),with statistically significant differences(P<0.05).Between groups,the study group showed significantly lower levels of systolic and diastolic blood pressure,LDL-C,TC,TG,LVEDD,LVESD,WBV,PAR,PV,and FIB compared to the control group,while HDL-C and LVEF were higher in the study group than in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion The use of Tongxinluo capsules in conjunction with atorvastatin calcium tablets,compared to atorvastatin calcium tablets alone,is more effective in improving blood pressure and lipid levels,as well as enhancing cardiac function and hemorheological parameters in elderly patients with coronary heart disease,demonstrating better clinical efficacy.
