摘要
目的:探讨超踝关节外固定架联合负压封闭引流(VSD)对开放性踝关节骨折患者疼痛、关节功能和骨折愈合的影响。方法:纳入2022年5月—2023年12月在涟水县人民医院接受治疗的68例开放性踝关节骨折患者,根据随机数字表法分为对照组(常规手术清创结合接骨板内固定治疗)和观察组(应用超踝关节外固定架联合VSD治疗),各34例。对比两组骨折恢复指标、疼痛应激指标、关节功能改善情况、并发症发生率。结果:观察组肿胀消退、创面愈合、骨折愈合、完全负重时间均短于对照组(P<0.05)。术后,两组视觉模拟评分法评分及血清皮质醇、总抗氧化能力水平降低,观察组低于对照组(P<0.05);术后,两组功能、疼痛、活动度评分升高,观察组高于对照组(P<0.05)。观察组并发症发生率低于对照组(P=0.031)。结论:超踝关节外固定架联合VSD可减轻开放性踝关节骨折患者疼痛程度,加快关节功能恢复和骨折愈合,安全性较高。
Objective:To investigate the effects of superankle external fixator combined with negative pressure closed drainage(VSD)on pain,joint function and fracture healing in patients with open ankle fracture.Methods:A total of sixty-eight patients with open ankle fracture who received treatment in Huai'an Lianshui County People's Hospital from May 2022 to December 2023 were included and divided into the control group(conventional surgical debridement combined with internal fixation with bone plate)and the observation group(superankle external fixator combined with VSD treatment)according to random number table method,with thirty-four cases.Fracture recovery index,pain stress index,improvement of joint function and complication rate were compared between the two groups.Results:The time of swelling regression,wound healing,fracture healing and complete weight-bearing in observation group were shorter than those in control group(P<0.05);after operation,both groups showed a decrease in visual analog scoring,serum cortisol,and total antioxidant capacity levels,with the observation group being lower than the control group(P<0.05);after operation,the scores of function,pain and motion in both groups were increased,and the observation group was higher than the control group(P<0.05);the complication rate of observation group was lower than that of control group(P=0.031).Conclusion:Superankle external fixator combined with VSD can reduce the pain of patients with open ankle fracture,accelerate the recovery of joint function and fracture healing,with high safety.
