摘要
目的探究经关节置换术治疗膝关节退行性骨关节炎老年患者的疗效。方法随机选取2022年10月—2023年10月赫章县人民医院收治的80例老年膝关节退行性骨关节炎患者为研究对象,按照不同治疗方法分为参比组和观察组,各40例。其中参比组予以保守治疗,观察组在参比组的基础上予以关节置换术治疗。对比两组患者的治疗效果、膝关节功能、骨关节炎情况、日常生活活动能力、并发症情况等。结果观察组治疗总有效率95.00%(38/40)高于参比组的75.00%(30/40),差异有统计学意义(χ^(2)=6.275,P=0.012)。治疗后,两组患者的膝关节活动度、美国特种外科医院膝关节评分、日常生活能力量表评分均高于治疗前,关节炎指数评分低于治疗前,且观察组膝关节活动度、美国特种外科医院膝关节评分、日常生活能力量表评分均高于参比组,关节炎指数评分低于参比组,差异有统计学意义(P均<0.05)。观察组并发症总发生率低于参比组,差异有统计学意义(P<0.05)。结论经关节置换术予以老年膝关节退行性骨关节炎患者手术治疗效果明确,既可减轻患者骨关节炎症状,又可改善患者膝关节功能、日常生活活动能力,且安全性更高。
Objective To explore the curative effect of joint replacement in the treatment of senile patients with knee degenerative osteoarthritis.Methods A total of 80 elderly patients with degenerative osteoarthritis of knee joint admit⁃ted to Hezhang County People's Hospital from October 2022 to October 2023 were randomly selected as the research objects.According to different treatment methods,they were divided into reference group and observation group,with 40 cases in each group.The reference group was treated with conservative treatment,and the observation group was treated with joint replacement on the basis of the reference group.The therapeutic effect,knee joint function,osteoar⁃thritis,activities of daily living and complications were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.00%(38/40)higher than 75.00%(30/40)in the reference group,and the difference was statistically significant(χ^(2)=6.275,P=0.012).After treatment,the knee joint activity,knee joint score of the United States Special Surgery Hospital and daily living ability scale score of the two groups were higher than those before treatment,and the arthritis index score was lower than that before treatment,the knee joint activity,knee joint score of the United States Special Surgery Hospital and daily living ability scale score of the observation group were higher than those of the reference group,and the arthritis index score was lower than that of the reference group,the differences were statistically significant(all P<0.05).The total incidence of complications in the observation group was lower than that in the reference group,and the difference was statistically significant(P<0.05).Conclusion The surgical treatment of elderly patients with degenerative osteoarthritis of knee joint by joint replacement is effective,which can not only reduce the symptoms of osteoarthritis,but also improve the knee joint function and daily liv⁃ing ability of patients,and has higher safety.
