摘要
目的研究乳痈消方治疗肿块期非哺乳期乳腺炎(NPM)对患者症状和实验室检测指标的改善效果。方法以2020年8月—2022年4月浙江中医药大学附属丽水中医院收治的88例肿块期NPM患者随机分为西药组(44例)和联合中药组(44例)。西药组采用甲磺酸溴隐亭进行治疗,联合中药组在西药组的基础上采用乳痈消方进行治疗,两组均治疗4周。统计两组治疗4周后的疗效,比较两组治疗前和治疗4周后症状体征积分、实验室检测指标、炎症指标。结果治疗4周后,联合中药组总有效率高于西药组(P<0.05)。治疗4周后与治疗前比较,两组乳房脓肿、乳房肿物、乳房瘘管、乳房红肿、乳房疼痛、乳头凹陷评分及血清免疫球蛋白A(IgA)、总补体(TC)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、外周血白细胞(WBC)、中性粒细胞(N)、自然杀伤细胞(NK)水平降低,联合中药组乳房脓肿、乳房肿物、乳房瘘管、乳房红肿、乳房疼痛、乳头凹陷评分及血清TC、IL-6、TNF-α、CRP、外周血WBC、N、NK水平低于西药组(P<0.05)。治疗4周后与治疗前比较,两组外周血CD_(4)^(+)水平升高,联合中药组高于西药组(P<0.05)。结论乳痈消方治疗肿块期NPM,可有效改善患者症状及实验室指标,调节机体免疫功能,降低机体炎症反应,具有较好的治疗效果。
Objective To study the effect of Ruyongxiao Recipe(乳痈消方)on improving symptoms and laboratory testing index of patients with lump stage non-lactation mastitis(NPM).Methods From August 2020 to April 2022,88 patients with NPM in lump stage were treated in Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University as the research objects,and they were randomly divided into the Western medicine group(44 cases)and the united traditional Chinese medicine group(44 cases).The Western medicine group was treated with bromocriptine mesylate,and the united traditional Chinese medicine group was treated with Ruyongxiao Recipe on the basis of the Western medicine group.Both groups were treated for 4 weeks.The curative effect of the two groups after 4 weeks of treatment was counted,and the symptoms and signs scores,laboratory test indexes and inflammation indexes of the two groups before and after 4 weeks of treatment were compared.Results After 4 weeks of treatment,the total effective rate of the united traditional Chinese medicine group was higher than that of the Western medicine group(P<0.05).After 4 weeks of treatment,compared with those before treatment,the scores of breast abscess,breast tumor,breast fistula,breast swelling,breast pain,nipple depression,levels of serum immunoglobulin A(IgA),total complement(TC),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and peripheral white blood cells(WBC)decreased in both groups.The scores of breast abscess,breast tumor,breast fistula,breast swelling,breast pain,nipple depression,levels of serum TC,IL-6,TNF-α,CRP,WBC,neutrophils(N)and natural killer cells(NK)in the united traditional Chinese medicine group were lower than those in the Western medicine group(P<0.05).After 4 weeks of treatment,compared with those before treatment,the level of CD_(4)^(+) in peripheral blood of the two groups increased,and the united traditional Chinese medicine group was higher than the Western medicine group(P<0.05).Conclusion Ruyongxiao Recipe could effectively improve the symptoms and laboratory indexes of patients,regulate the immune function and reduce the inflammatory reaction of the body,and had a good therapeutic effect.
