摘要
目的:本研究观察中药内服外用治疗非哺乳期乳腺炎的临床疗效。方法:将108例确诊为非哺乳期乳腺炎的患者,其中早期急性期54例、术后缓解期54例,分别随机分为抗生素组、中药内服组、中药内服+外用组,每组各18例。抗生素组给予头孢美唑治疗后观察,中药内服组给予阳和汤加减汤剂,中药内服+外用组在给予阳和汤加减汤剂基础上加用金黄散外敷,各组患者于治疗后6个月评价疗程及疗效。结果:无论在早期急性期还是术后缓解期,与抗生素相比,中药以及中药+外用治疗均可显著缩短非哺乳期乳腺炎的治疗周期;中药、中药+外用治疗在改善乳房疼痛、肿块大小、红肿情况方面,均较抗生素具有明显的优势;在非哺乳期乳腺炎的急性期,与抗生素相比,中药+外用治疗可明显提高治疗有效率;而在非哺乳期乳腺炎的术后缓解期,中药、中药+外用组不但治疗有效率明显高于抗生素组,同时还显著缩短溃口引流时间、降低复发率。结论:中药内服外用在缩短非哺乳期乳腺炎疗程,改善急性期症状,促进术后缓解期预后,降低复发率等方面具有良好的治疗作用。
Objective:To observe the clinical efficacy of applying herbal internal and external administration in the treatment of non-puerperal mastitis.Methods:A total of 108 patients who were diagnosed with non-puerperal mastitis,including 54 cases in early acute stage and 54 cases in postoperative remission stage,were enrolled in the study and each stage randomly divided into 3 groups,the antibiotics groups,the TCM groups and the oral combined with topical therapy of TCM groupwhich per group had 18 patients respectively.The Cefmetazole were given to the antibiotic groups,the Modified Yanghe Decoction were given to the TCM group and the Modified Yanghe Decoction added to Ruyi Golden Powder were given to the internal and external administration of TCM groups.We evaluated the efficiency after the treatment of 6 months.Results:Both in the early acute stage and postoperative remission stage,compared with antibiotics,TCM and TCM+topical therapy can significantly shorten the treatment cycle of non-puerperal mastitis.Both the TCM and TCM+topical therapy showed prominently remission on breast pain,size of masses,breast red and swollen than the antibiotics.In the acute stage,TCM+topical treatment significantly improved the therapeutic effect.In postoperative remission,the effective rate of TCM and TCM+topical therapy were observably higher than that of antibiotics,the drainage duration time was remarkably shortened and the recurrence rate was notably reduced.Conclusion:Internal and external use of TCM had a good therapeutic effect on non-puerperal mastitis,especially on shortening the course of treatment,allivating acute symptoms,promoting the prognosis of postoperative remission and reducing the recurrence rate.
