摘要
慢性肾小球肾炎以蛋白尿、血尿、高血压、水肿为基本临床表现。其中高血压作为肾性高血压的一种,西医常用ACEI/ARB、利尿剂、β受体阻滞剂(β-blocker)、钙通道阻滞剂(CCB)、α受体阻滞剂等几类药物联合应用进行治疗,但是其不良反应大,价格昂贵,降压效果不理想。当前,针对影响血压变化的因素研究中,肾素-血管紧张素-醛固酮系统(RAAS)是被国内外学者研究最为透彻的一种,主流降压药也大都作用于该系统,血管紧张素原被此系统的PRA转化成AngI,在血管紧张素转换酶(ACE)的催化下,AngI进一步被转化成AngII,刺激ALD分泌。ALD、AngII、PRA具有强大的缩血管功能,可以引起血压升高造成高血压。郭恩绵教授通过研究中医经典结合大量临床观察,发现以自拟方玉肾露联合天麻钩藤饮加减治疗具有显著的效果。
The basic clinical manifestations of chronic glomerulonephritis are proteinuria,hematuria,hypertensionand edema.Among them,hypertension is a kind of renal hypertension.Western medicine commonly uses angiotensin converting enzyme inhibitor(ACEI)/angiotensin II receptor blocker(ARB),diuretics,β-blocker,calcium channel blocker(CCB),α-receptor blocker,etc.However,it has significant side effects,high priceand poor blood pressure reduction effect.Currently,in the study of factors affecting blood pressure changes,therenin-angiotensin-aldosterone system(RAAS)is the most thoroughly studied by scholars at home and abroad,and most mainstream antihypertensive drugs also act on this system.Angiotensinogen is converted into AngI by the PRA of this system,and AngI is further converted into AngII under the catalysis of angiotensin converting enzyme(ACE),stimulating the secretion of ALD.ALD,AngIIand PRA have strong vasoconstrictive effects,which can cause blood pressure elevation and hypertension.Professor GUO Enmian has found that the self-made Yushen Lu(玉肾露)combined with Tianma Gouteng Yin(天麻钩藤饮)have significant effectsthrough clinical observation.
