摘要
目的 描述和分析中国0~19岁儿童青少年抑郁现状及其疾病负担的长期变化趋势。方法 数据来源于2019年全球疾病负担研究,利用Joinpoint回归模型分析抑郁发病率及伤残调整生命年(disability adjusted of life years,DALYs)长期变化趋势,使用线性模型计算平均年度变化百分比(average annual percent change,AAPC)。结果 2019年中国儿童青少年抑郁发病率[644.78/10万(95%UI:481.33/10万~855.00/10万)]和DALYs明显低于全球平均水平[1 233.16/10万(95%UI:915.64/10万~1 654.63/10万)]。1990-2019年我国儿童青少年总体抑郁发病率和DALYs下降,其中<10岁儿童发病率和DALYs呈上升态势(P<0.05)。1990-2019年中国儿童青少年抑郁发病率和DALYs的AAPC分别为-1.33%[95%置信区间(confidenceinterval, CI):-1.53%~-1.13%, P<0.01]和-1.10%(95%CI:-1.29%~-0.91%,P<0.01)。结论 1990-2019年中国儿童青少年抑郁发病率和DALYs整体呈下降趋势,女性抑郁发病率远高于男性,15~19岁发病率高于其他年龄段,应加强关注儿童青少年抑郁问题。
Objective To describe and analyze the status quo and long-term trend of the disease burden of depression in children and adolescents aged 0 to 19 years in China.Methods The relevant data were obtained from the 2019 Global Burden of Disease study.The secular trend of incidence rate and disability-adjusted life years(DALYs)were analyzed using Joinpoint regression models,and average annual percent change(AAPC)estimated using linear models.Results In 2019,the incidence and DALYs of depression[644.78/100000(95%UI:481.33/100000-855.00/100000)]in children and adolescents in China were significantly lower than the global average levels[1233.16/100000(95%UI:915.64/100000-1654.63/100000)].From 1990 to 2019,the overall incidence and DALYs of depression in children and adolescents in China decreased,except for an increasing trend of both incidence and DALYs in children under 10 years(P<0.05).From 1990 to 2019,the AAPC in the incidence and DALYs of depression in children and adolescents in China were−1.33%(95%CI:−1.53%-−1.13%,P<0.01)and−1.10%(95%CI:−1.29%-−0.91%,P<0.01),respectively.Conclusions The incidence and DALYs of depression in children and adolescents in China showed an overall decreasing trend in the period from 1990 to 2019.The incidence of depression in females far exceeds that in males,and the incidence is much higher in 15-to 19-year-olds than in other age groups.It is urgent to pay more attention to mental health of children and adolescents.
