摘要
目的运用数据挖掘技术分析阳陵泉现代主治优势病症及腧穴配伍规律。方法检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、中国学术期刊数据库(万方数据)、中国生物医学文献数据库(CBM)、PubMed、Web of Science及Embase建库至2022年12月31日收录的阳陵泉相关研究文献。建立阳陵泉单穴、配伍主治病症和配伍腧穴等数据库,利用Gephi0.10.1、SPSS Modeler 18.0、Cytoscape3.9.1、SPSS27.0软件对纳入文献进行复杂网络分析、关联分析和聚类分析。结果纳入文献530篇,阳陵泉单穴主治病症37种,优势病症为肩关节周围炎、落枕、急性胆囊炎及踝关节扭伤等,治疗方式以运动针法为主;配伍主治病症105种,优势病症为膝骨关节炎、腰椎间盘突出症及偏头痛等24种,治疗方式以电针为主。阳陵泉高频配伍特定穴为五输穴、交会穴、下合穴,高频配穴为足三里、血海、三阴交、阴陵泉、犊鼻、内膝眼等17个腧穴,配伍经脉主要为足阳明胃经,聚类分析共得3系4类配伍处方组合。结论阳陵泉主治范围广泛,以运动系统病症为主,单穴、配伍应用各有优势病症。特定穴多重配伍与聚类处方灵活组合,有助于临床精准选穴、科学用穴。
Objective To analyze the modern dominant indications and compatibility regularity of Yanglingquan(GB34)using data mining technique.Methods The literature from the establishment of the databases to 31,Dec.2022 in CNKI,VIP,Wanfang Data,CBM,PubMed,Web of Science and Embase were used as search sources to establish the database for single acupoint,dominant indications and compatible acupoints of GB34.With the aid of Gephi 0.10.1,SPSS Modeler 18.0,Cytoscape 3.9.1 and SPSS 27.0,complex network analysis,association analysis,and clustering analysis were performed.Results A total of 530 articles were included.There were 37 kinds of singleacupoint indications,among which the scapulohumeral periarthritis,stiff neck,acute cholecystitis and sprain of ankle joint etc.were dominant ones,the treatment was mainly sports acupuncture.There were 105 kinds of compatible acupoints indications,and the 24 dominant ones were knee osteoarthritis,lumbar disc herniation,and migraine,etc.,the treatment was mainly electroacupuncture.The compatible acupoints of GB34 were mainly specific acupoints such as the Five-shu acupoints,the crossing acupoints,the Xia-he acupoints.There were 17 high-frequented used compatible acupoints including Zusanli(ST36),Xuehai(SP10),Sanyinjiao(SP6),Yinlingquan(SP9),Dubi(ST35),Neixiyan(EX-LE4),etc.The stomach meridian was the one with highest frequency among the compatible meridians.Clustering analysis obtained 3 clusters of 4 kinds of compatible prescriptions.Conclusion GB34 has a wide range of indications,mainly in motor system diseases.Single acupoint and compatible application have their own core dominant indications,while the flexible combination of multiple compatibility and clustering prescription of specific acupoints is helpful to provide reference in guiding accurate acupoint selection and scientific application in clinical practice.
