摘要
目的基于数据挖掘探索针刺治疗顽固性失眠的临床选穴规律和特点。方法检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(万方数据)、维普中文期刊服务平台(VIP)、中国生物医学文献服务系统(SinoMed)和PubMed建库至2021年11月24日收录的针刺治疗顽固性失眠临床研究文献,建立针刺处方数据库,采用SPSS软件进行关联规则分析及系统聚类分析。结果共纳入106篇文献,提取112首针刺处方,涉及腧穴67个,总频次为688次。使用频次较高的腧穴为神门、三阴交、百会;腧穴较多归督脉和足太阳膀胱经;部位多涉及头面颈部和四肢部;特定穴以五输穴、原穴、八脉交会穴较多;关联规则分析支持度较高的有神门-内关、神门-内关-三阴交、神门-百会-三阴交;聚类分析得到3个聚类群。结论针刺治疗顽固性失眠选穴以头面颈部分布较多,处方以特定穴为主,经外奇穴也有涉及,符合临床治疗规律。
Objective To explore the clinical acupoint selection law and characteristics of acupuncture treatment for intractable insomnia based on data mining.Methods Clinical research literature about acupuncture for intractable insomnia was retrieved from CNKI,Wanfang Data,VIP,SinoMed and PubMed from the establishment of databases to November 24,2021,and acupuncture prescription database was established.SPSS software was used for association rule analysis and system clustering analysis.Results A total of 106 articles were included,and 112 acupuncture prescriptions were extracted,involving 67 acupoints,with a total frequency of 688 times.The high frequently used acupoints were Shenmen(HT7),Sanyinjiao(SP6),and Baihui(DU20);most acupoints belonged to governor vessel and bladder meridian of foot-taiyang;most of the acupoints involved the head,face,neck and limbs;specific point to five-shu points,primary points,and eight confluence points more;the association rule analysis with high support was Shenmen(HT7)-Neiguan(PC6),Shenmen(HT7)-Neiguan(PC6)-Sanyinjiao(SP6),Shenmen(HT7)-Baihui(DU20)-Sanyinjiao(SP6);3 clusters were obtained by cluster analysis.Conclusion Acupuncture points for intractable insomnia are mostly distributed on the head,face and neck.The prescriptions are mainly specific point,and extra points are also involved,which is in line with the clinical treatment rules.
