摘要
目的 研究经阴道三维超声(3D-TVS)血流参数对宫腔粘连(IUA)患者宫腔镜下宫腔粘连分离术(TCRA)手术效果的评估价值。方法 纳入IUA患者78例,所有患者均行TCRA治疗,并于术后3月记录IUA患者的TCRA疗效,将治疗有效者纳入有效组,无效者纳入无效组。比较术前和术后3个月2组3D-TVS指标,包括子宫内膜容积(EV)和血流参数[血流指数(FI)、血管指数(VI)以及血管血流指数(VFI)]水平。使用受试者工作特征(ROC)曲线分析术后3D-TVS指标对IUA患者手术疗效的评估价值。结果 将治愈和好转患者55例纳入有效组,无效23例纳入无效组。术后,有效组的EV、FI、VI以及VFI均高于术前,且均优于无效组,差异具有统计学意义(均P<0.05)。结论 3D-TVS中FI、VI以及VFI可作为评估IUA患者TCRA疗效的有效指标。
Objective To study the evaluation value of transvaginal three-dimensional ultrasonography(3D-TVS)blood flow parameters in the outcomes of trans-cervical resection of adhesions(TCRA)of patients with intrauterine adhesions(IUA).Methods Seventy-eight IUA patients who received TCRA treatment were enrolled.The TCRA effect on IUA patients was recorded 3 months after the operation,the effective patients were included in the effective group,and the ineffective patients were included in the ineffective group.The levels of 3D-TVS indexes were compared before and 3 months after the operation in the 2 groups,including EV and blood flow parameters(FI,VI,and VFI).The receiver operator characteristic curve(ROC)was used to analyze the evaluation value of postoperative 3D-TVS for the surgical efficacy of patients with IUA.Results Fifty-five cases of cured and improved patients were included in the effective group,and 23 cases of ineffective patients were included in the ineffective group.After the operation,EV,FI,VI,and VFI in the effective group were higher than those before the operation and were better than those in the ineffective group(P<0.05).Conclusion FI,VI,and VFI in 3D-TVS can be used as effective indicators to evaluate TCRA efficacy in patients with IUA.
