摘要
子宫内膜异位症不孕主要归属于传统医学“无子”范畴,根据不同的临床表现,又可称之“癥瘕”“经行腹痛”等。国内外资料显示,当前有将近50%子宫内膜异位症(内异症)患者合并不孕,如何有效提高内异症相关性不孕患者妊娠率已成为目前妇科疾病研究领域亟需解决的难题。近代中医认为该病以血瘀为核心病机,治疗上仍以三棱丸(《经验良方》)、桂枝茯苓丸(《金匮要略》)等活血方为主,配合针灸等疗法,沿用至今,临床收效显著。龙江韩氏妇科提出活血调肝补肾法治疗内异症不孕,其经验方内异止痛方合补肾活血调冲汤,联合药灸治疗该病,临床效果满意。该文搜集了近年来内异症不孕相关的文献资料,就子宫内膜异位症不孕的中医研究近况,结合龙江韩氏妇科特色诊疗经验,围绕该病病机和治疗做一综述。
Endometriosis infertility falls under the category of“no child”in traditional Chinese medicine.Depending on its clinical manifestations,it could be described as“lump”or“dysmenorrhea”.The data at home and abroad shows that nearly 50%of patients with endometriosis are complicated with infertility.How to effectively improve the pregnancy rate of endometriosis infer-tility patients has become an urgent problem to be solved in the field of gynecological diseases.Modern traditional Chinese medi-cine believes that blood stasis is the core pathogenesis of the disease,and Sanleng Pills(三棱丸),Guizhi Fuling Pill(桂枝茯苓丸)and other blood-activating prescriptions are still the main formulae,combined with acupuncture and moxibustion and other therapies,still in use today,and the clinical effects are remarkable.Longjiang Han's gynecology has proposed the method of acti-vating blood,soothing liver and tonifying kidney to treat endometriosis infertility.The experiential formula is Neiyi Zhitong Decoc-tion(内异止痛方)and Bushen Huoxie Tiaochong Decoction(补肾活血调冲汤)with combines moxibustion,receives good clini-cal effects.In this paper,the literature related to endometriosis infertility in recent years was collected,and the research status of endometriosis infertility in TCM was reviewed.The pathogenesis and treatment of this disease were summarized based on the diag-nosis and treatment experience of Longjiang Han's gynecology.
