肺部超声在儿童肺炎动态监测中的应用价值 被引量:2

Application value of pulmonary ultrasound in dynamic monitoring of pediatric pneumonia

摘要 目的探讨肺部超声在动态监测儿童肺炎中的应用价值。方法选择717例临床确诊肺炎的患儿,对其中不愿意行肺部超声动态监测的家属进行问卷调查了解原因,对进行肺部动态监测(3次肺部超声)的肺炎患儿(76例)进行胸部X线检查及不同治疗时期肺部超声检查,比较胸部X线检查和肺部超声检查的准确率及不同治疗时期肺部超声征象的情况。结果717例临床确诊肺炎的患儿中有416例患儿不接受肺部超声检查,有225例患儿治疗期间仅在治疗后期进行1次肺部超声检查,主要原因依次是检查费用高、多次检查麻烦,对检查项目不了解不信任以及认为检查没有必要;与胸部X线检查准确率(92.10%)比较,肺部超声检查准确率(96.05%),差异无统计学意义(χ^(2)=1.063,P>0.05);治疗前、中、后期肺部超声征象中胸膜线异常、A线消失、B线出现、肺实变比例比较,差异有高度统计学意义(P<0.001);不同治疗时期胸腔积液比例比较,差异无统计学意义(P>0.05)。结论肺部超声诊断儿童肺炎准确率高、副作用小、可多次检查,对动态监测儿童肺炎具有一定的应用价值。 Objective To investigate the application of pulmonary ultrasound in clinical practice and its application value in dynamic monitoring of pneumonia in children.Methods A total of 717 children with clinically diagnosed pneumonia were chosen,and the family members who were unwilling to undergo dynamic pulmonary ultrasound monitoring were investigated by questionnaire.The 76 children with pneumonia who underwent dynamic pulmonary monitoring(3 times lung ultrasound)were examined by chest X-ray and pulmonary ultrasound at different treatment periods.The accuracy of chest X-ray check and pulmonary ultrasound check,as well as the pulmonary ultrasound signs in different treatment periods were compared.Results Four hundred and sixteen out of 717 children with clinically diagnosed pneumonia refused pulmonary ultrasound check,and 225 subjects only received such check once in the late stage of treatment.The main reasons were high cost,trouble of multiple examinations,ignorance of such check and considering the check as unnecessary.Compared with chest X-ray check accuracy(92.10%),pulmonary ultrasound check accuracy(96.05%)had no significant difference(χ^(2)=1.063,P>0.05).There were statistically significant differences in the rates of abnormal pleural line,disappearance of line A,appearance of line B and detection of lung solid signs in pulmonary ultrasound before,during and after treatment(P<0.001).Pleural effusion comparison of different treatment stages showed no statistical significance(P>0.05).Conclusion Pulmonary ultrasound diagnosis of pediatric pneumonia shows high accuracy,few side effects and can be checked for multiple times,which shows certain application value for dynamic monitoring of pediatric pneumonia.
