摘要
目的:分析白藜芦醇协同多黏菌素B对碳青霉烯类耐药肠杆菌目细菌的体外抗菌效应。方法:从临床分离获得100株碳青霉烯类耐药肠杆菌目细菌,其中,耐碳青霉烯肠杆菌(CRE)38株,耐碳青霉烯铜绿假单胞菌(CRPA)22株,耐碳青霉烯类鲍曼不动杆菌(CRAB)40株。用肉汤稀释法测多黏菌素B单独与联合白藜芦醇时最低抑菌浓度(MIC),用棋盘试验计算部分抑菌浓度指数(FIC),判定多黏菌素B与白藜芦醇之间是否存在协同作用。结果:各菌株对白藜芦醇的MIC值均在512μg/mL以上;加入白藜芦醇前,100个菌株中多黏菌素B的MIC值≥4μg/mL的有90株,其他10个菌株为2μg/mL;加入白藜芦醇后,分离株中针对多黏菌素B的MIC显著降低。在CRE,CRPA,CRAB三类菌株中,白藜芦醇均可发挥相似作用。15株细菌菌株FIC指数均小于0.5,表明多黏菌素B和白藜芦醇之间存在协同效应。不同浓度多黏菌素B协同128μg/mL白藜芦醇时,其抑菌率与单独应用多黏菌素B比较,明显升高。结论:白藜芦醇协同多黏菌素B对碳青霉烯类耐药肠杆菌目细菌可发挥较好抗菌作用。
Objective:To analyze the in vitro antibacterial effect of resveratrol and polymyxin B on carbapenem-resistant Enterobacteriaceae.Methods:A total of 100 carbapenem-resistant Enterobacterales were isolated from clinical specimens,including 38 carbapenem-resistant Enterobacteriaceae(CRE),22 carbapenem-resistant Pseudomonas aeruginosa(CRPA),and 40 carbapenem-resistant Acinetobacter baumannii(CRAB).The minimum inhibitory concentration(MIC)of polymyxin B alone or in combination with resveratrol was determined by broth dilution method.Checkboard test was used to calculate the partial inhibitory concentration index(FIC)to determine whether there was synergistic effect between polymyxin B and resveratrol.Results:The mics of all strains to resveratrol were above 512μg/mL.Before resveratrol was added,the mics of 90 strains to polymyxin B were≥4μg/mL,and the mics of the other 10 strains were 2μg/mL.The MIC against polymyxin B was significantly reduced in the isolates after the addition of resveratrol.Resveratrol played a similar role in CRE,CRPA and CRAB strains.FIC was less than 0.5 in all 15 bacterial strains,indicating a synergistic effect between polymyxin B and resveratrol.The inhibition rate of different concentrations of polymyxin B+128μg/mL resveratrol was significantly higher than that of polymyxin B alone.Conclusion:Resveratrol combined with polymyxin B has a good antibacterial effect against carbapenem-resistant enterobacterales.
