摘要
消化性溃疡(peptic ulcer,PU)是一种多发于胃和十二指肠的慢性溃疡,以反复的周期性、节律性上腹部隐痛不适为主要临床表现,常伴有神经功能综合征。其病因复杂多样,难以自愈,是消化科门诊中的常见病种。目前PU发病机制包括幽门螺杆菌感染、遗传因素、社会应激等,但仍未完全明确。近年来大量研究显示精神情志因素在PU的发病中占据着重要地位,“心藏神”表明心统帅人体的思维与精神活动,“心与小肠相表里”是中医经典理论之一,心与小肠通过经脉属络互为表里,生理相依,病理相及,二者共同调摄胃肠的受纳、腐熟以及受盛化物功能,这与现代西医提出的心-脑-肠轴理论具有一致性。在临床中,从心与小肠的关系来论治消化性溃疡也越来越常见,和肠清热、和肠止痛等结合养心、宁心、通心等治法取得了显著疗效。该文将中西医理论有机地结合来论述PU,以期为中西医结合防治PU提供一些思路。
Peptic ulcer(PU)is a chronic ulcer occurring in the stomach and duodenum.It is characterized by recurrent periodic and rhythmic pain and discomfort in the upper abdomen,often accompanied by neurological syndrome.Its etiology is complex and diverse,and it is difficult to heal itself.It is a common disease in gastroenterology clinic.At present,the pathogenesis of PU includes Helicobacter pylori infection,genetic factors and social stress,but it is still not completely clear.In recent years a large number of studies have shown that the spirit of modern factors in the onset of PU occupies the important position,the“heart”god showed that heart in thinking and mental activities of the human body,“heart and small intestine”to the vocabulary is one of the classical theory of traditional Chinese medicine,the heart and small intestine through the meridians collaterals are inside,the physiological dependence,pathology,and the two common cultivate the acception of gastric bowel,rotten,and the compound function.This is consistent with the theory of the heart-brain-gut axis proposed by modern Western medicine.In clinical practice,it is more and more common to treat peptic ulcer from the relationship between the heart and the small intestine,The combination of harmonizing intestine and clearing heat,harmonizing intestine and relieving pain and nourishing the heart,calming the heart,and soothing the heart had achieved significant therapeutic effects.In this paper,the theory of traditional Chinese and Western medicine is organically combined to discuss PU,in order to provide some ideas for the prevention and treatment of PU.
