摘要
目的探讨T2DM患者甘油三酯葡萄糖乘积(TyG)指数、TG/HDL⁃C比值与超声心动图参数的相关性,分析TyG指数及TG/HDL⁃C对左心室重构的预测价值。方法选取2020年9月至2021年12月于南京医科大学附属常州第二人民医院内分泌代谢科治疗的T2DM患者241例,根据TyG指数及TG/HDL⁃C的四分位数分为Q1、Q2、Q3、Q4共4组,分析TyG指数、TG/HDL⁃C与超声心动图参数的相关性,受试者工作特征(ROC)曲线分析TyG指数及TG/HDL⁃C对左心室重构的诊断价值。结果Q4组超声心动图参数高于Q1组(P<0.05)。Spearman相关分析显示,TyG指数、TG/HDL⁃C与左心室质量指数(LVMI)、左心室质量(LVM)、左心房内径(LAD)呈正相关(P<0.05),与左心室射血分数呈负相关(P<0.05)。多元线性回归分析显示,TyG指数是LVMI、LVM、LAD的危险因素,TG/HDL⁃C是LVEF的危险因素。ROC曲线分析显示,TyG指数可作为预测左心室重构的指标,最佳临界值为9.21,与TG/HDL⁃C联合诊断预测价值更高,准确率为74.19%。结论TyG指数、TG/HDL⁃C与T2DM患者左心室重构密切相关,对左心室重构有一定预测价值。
Objective To explore the correlation between triglyceride⁃glucose(TyG)index and TG/HDL⁃C and echocardiographic parameters in patients with type 2 diabetes mellitus(T2DM),and to analyze the predictive value of TG/HDL⁃C and TyG index on left ventricular remodeling.Methods 241 patients with T2DM were recruited in the Endocrine Metabolism Department of Changzhou Second People's Hospital Affiliated to Nanjing Medical University from September 2020 to December 2021.Patients were divided into four groups(Q1~Q4)according to the quartiles of TyG index and TG/HDL⁃C.Results The echocardiographic indices in Q4 group were significantly higher than that in Q1 group(P<0.05).Spearman correlation analysis showed that both TyG index and TG/HDL⁃C were positively correlated with LVMI,LVM and LAD(P<0.05)and negatively correlated with LVEF(P<0.05).Multiple linear regression analysis showed that TyG index was a risk factor for LVMI,LVM and LAD,and TG/HDL⁃C was a risk factor for LVEF.ROC curve analysis showed that TyG index can be used as a predictor of ventricular remodeling with an optimal threshold value of 9.21,and has a higher predictive value in combination with TG/HDL⁃C,with an accuracy of 74.19%.Conclusion Both TyG index and TG/HDL⁃C are closely associated with left ventricular remodeling.The combination of TyG index and TG/HDL⁃C has predictive value for left ventricular remodeling inT2DM patiens.
